A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. FCC Emergency Broadcasting placard in the operating station was deteriorated. 46 CFR 184.506/ 16C.

Each nozzle must be of corrosion-resistant material and be capable of being changed between a solid stream and a spray pattern. The nozzle was fixed of a solid steam, not capable of spray pattern. 46 CFR 181.320 (d)/ 16C

Watertight doors and watertight hatches must be marked on both sides in clearly legible letters at least 1 inch high: “WATERTIGHT HATCH—KEEP CLOSED”. Forward collision bulkhead watertight hatch does not have required markings.46 CFR 185.610/ 16C.

Each hatch exposed to the weather must be watertight. Forward collision bulkhead hatch was found leaking profusely, and it was adjusted. However, minimal leak is still present due to faulty rubber gasket. 46 CFR 174.220 (a)/ Code 17/re-classified as 16C.

The owner of a vessel must designate a name for the vessel on the Application for Initial Issue, Exchange, or Replacement of Certificate of Documentation; or Redocumentation (form CG-1258) submitted to the Director, National Vessel Documentation Center when the owner elects to change the name of the vessel. The vessel name in the Certificate of Documentation is "LADECIMA" (one word, no space), and the vessel name both in Certificate of Inspection and in the transom is "La Decima" (two words, with space in between). Either correct the vessel name with the NVDC, or correct the vessel name in the transom, and notify USCG Sector San Juan within 14 days of the decision and action taken. 46 CFR 67.111 (a)/ 16C.

Each hatch exposed to the weather must be watertight. Hatch cover located in the forward collision bulkhead was found leaking water profusely to the compartment upon testing. 46 CFR 174.220(a) Code 17C.

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. The EAP Placard was not available. 46 CFR 16.401(a). Code 16C.

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow and the stern of the vessel. The hailing port of the vessel must be marked on some clearly visible exterior part of the stern of the vessel. Vessel name is not displayed in port and starboard sides, not in the stern of the vessel. Also, the hailing port is not displayed. 46 CFR 67.123 Code 16C.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Minimize accidental personnel contact with energized parts; and Prevent electrical ignition of flammable vapors. Multiple dead-end wires Engine Room # 2 MDE and underneath the operating station console cabinet. 46 CFR 183.200 Code 16C.

A self priming, power driven fire pump must be installed on each vessel the fire pump must be capable of both remote operation from the operating station and local operations at the pump. Local controls to start the fire pump located in the steering compartment was found inoperable, and unable to energize the electric fire pump. 46 CFR 181.300 Code 16C.

All observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Two through hole penetrations in the engine room which is a CO2 protected space were found underneath the operating station through cables runs between the deck and the CO2 activation pull cables. 46 CFR 176.830 Code 16C.

The shafts, bearings and propellors were pulled.

Certificate of Documentation is expired.

Radio Certificates (Radio telephony etc.) were expired.

Fire fighting was last inspected in 2016.

Install metal clamps on the over head wires in the engine room.

Perform battery maintenance (corrosion)

Update expired medicine in first aid kit.

Replace corroded exhaust riser elbows connected to the turbochargers for both port & stbdside MDEs.

Prove proper operation of both bilge alarms and bilge pumps.

Clean and sanitize rodent feces found at operating station.

Vessel must undergo and complete a drydock inspection prior returning for passenger for hire service after missing drydock inspection due on 30APRIL2016.

Repair/replace rubber gastek and make engie access door airtight.

Show proof of an original Certificate of Documentation.

Make proper repairs to damaged forward portside shell in way of rubrail and provide repair proposal.

Remove vessel from water due to a vessel collision and make ready for damage survey by the satisfaction of an attending USCG Marine Inspector.

Make proper repairs to damaged to forward light tower, radar and bell.

Remove excess bilge water from engine room.

Repair portside aft bit due to fracture at weld.

Replace worn life ring grab line.

Repair or replace dead end wires underneath operating station.

Repair exhaust support brackets on both port and stbd side engines.

Prove proper operation for stern navigation light.

Provide proof of alcohol testing and DAPI programs.

Prove proper operation of fire boy system in accordance with manufacturer's instructions.

Prove proper operation for aft, stbd side, fire damper prior to carrying passengers.

Provide proof of crew training for abandon ship, man overboard, and fire fighting drills.

Vessel captain to prove knowledge and operation of fire boy system to the satisfaction of USCG inspector.

Make man overboard rescue ladder of appropriate length.

Provide Repair Proposal for stbdside strut, propellers and shaft.

Provide worklist proposal for credit drydock.

Provide repair proposal for stbdside strut and shaft, including port side propeller.

Provide worklist proposal for credit drydock.

Drydock vessel and make ready for inspection due to possible collision with underwater object.

Provide current navigation publications: i.e. tide tables, tidal current tables and U.S. Coastal Pilot.

Properly log training and drills on a vessel log and have log on board at all times.

Provide vessel safety orientation to passengers prior sailing or post a safety orientation placard visible to passengers.

Prior to carrying passengers, Dry Dock vessel and make ready for Coast Guard inspector.

Properly repair exhaust leak on port side engine exhaust piping next to water injection port. There was a temporary repair completed and accepted, but a permanent repair must be done to clear this deficiency.

Fix jacket water leak on port side engine.

Adress broken and loose straps on exhaust lagging for both port and starboard engines.

Provide proper placement of retro-reflective tape on lifering stbd side aft.
