Chemical tankers and gas carriers carrying cargoes emitting toxic vapors or gases shall carry in lieu of totally enclosed lifeboats complying with the requirements of section 4.8 of the code. PSCO observed air supply hose holed on P/S lifeboat, rendering the lifeboat air support system inoperative. SOLAS 74a (2001) Cons III/ 31.1.6 LSA code 4.8

The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship-based and shore-based. Vsl has had inoperable fuel tank high level alarm since Aug 2017 and was reported to the company but has not received a replacement. Vsl did not have an alternate procedure during internal transfer for the crew when fuel tank hi lvl alarm or a critical equipment is inoperable. These factors may have prevented the spill incident. ISM 2.2

In addition to the requirements in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration. The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to...other hazards. The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the Houston Ship Channel. The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The safety management system should provide for specific measures aimed at promoting the reliability of such equipment or systems. The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely impacted its fitness and capability to safely maneuver the Houston Ship Channel. Submit evidence to [email withheld] detailing how this hazardous situation was reported to the company, investigated, and analyzed with the objective of improving safety, including the implementation of corrective action and measures intended to prevent recurrence (as detailed and required in ISM A/9). 74SOLAS (2014 CONS.) 0330 Water closets shall be provided with ample flush of water, available at all times and independently controlled. Toilets did not flush upon several attempts. 33USC 1223(b)

0950 All electrical apparatus shall be so constructed and installed to not cause injury when handled or touched. Galley electrical outlet was supplied with power from a spliced extension cord plugged into another outlet. SOLAS 2001 CH II-1 REG 45.1.3

0610 Vessel's crew was unable to start the port lifeboat engine. All lifesaving appliances shall be in working order and ready for immediate use. SOLAS 2009 CH III REG 20.2 LSA CH IV 4.4.6.2

0710 As per fire control plan, various doors enclosing spaces of Class "A" divisions shall be self-closing. Multiple doors required to be self-closing did not self close. SOLAS 2001 CH II-2 Reg 20.1

2199 Harmful substances shall be properly stored and secured to minimize the hazards to ship’s crew and marine environment. Vessel had multiple tank cleaning chemicals stored on deck in a non-designated stowage location.

Vessels crew did not properly record routine inspections as required by EPA vessel general permit. the last entry found was Januarty 2011. VGP 4.1.1.1 [name withheld] [phone withheld] MSU Texas City, Texas

Vessel experienced a grounding while in the Mississippi River.

0610 - 7 portside lifeboat rations and 9 starboard side lifeboat rations were not in airtight packaging as required by code. SOLAS 97 Consol Ed Chpt III 8.12 & LSA Chpt III 4.4.8.12.

Vessel was found with three fire or explosion hazards in engine room.

The emergency generator failed to operate as required. During the course of the inspection the vessel's crew was unable to start the emergency generator. After trying for 20 minutes the emergency generator started but could not be restarted on a regular basis.

Fire [name withheld] indicates that life rings fitted with buoyant lifeline, also must be fitted with self ignition light. There were no self igniting lights.

The nonmetallic cargo hoses for hazardous materials were not marked properly.

Vessel request inbound deviation for inoperable Gyro Compass.
