The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship-based and shore-based. Vsl has had inoperable fuel tank high level alarm since Aug 2017 and was reported to the company but has not received a replacement. Vsl did not have an alternate procedure during internal transfer for the crew when fuel tank hi lvl alarm or a critical equipment is inoperable. These factors may have prevented the spill incident. ISM 2.2
