Each valve must close with a right-hand (clockwise) motion of the hand-wheel. Valves on the fuel oil pipeline to the thermal fluid heater and generator were found locked in the open position with zip ties which were unable to be broken by turning the valve handle.

A Coast Guard Marine Inspector noted that the vessel is equipped with a prime mover that is known to not be in compliance with the Class 1, Division 1 standards outlined in 46 CFR 111.105-3. The deadline for the implementation of Class 1, Division 1 compliant equipment was November 1, 2015. The cargo pump engine is located within a hazardous zone.
