Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, a door designed to re-open upon contacting an object in itss path shall reopen not more than 1m from the point of contact. Sliding FSD 09.7.03 had a crash bar that malfurnctioned and would not reset. 74 SOLAS 20 II-2/9.4.1.1.5.11

Following installation, the master, owner, operator, agent, or person in charge of the vessel must properly maintain the BWMS in accordance with all manufacturer specifications. Vessel's ballast water management system is inoperable. Vessel shall not discharge ballast water into U.S. waters. 33 CFR 151.2025(a)(1)

Waste receptacles shall be constructed of non-combustible materials with no openings in the side or bottom. PSCO observed plastic garbage cans or bags containing combustible materials in the main laundry space and the manager office main deck 6 aft. Additionally on the horizon court . PSCO observed garbage stowed on top of the designated container. 74SOLAS20 II-2/4.4.2

Fire doors in main vertical zones bulkhead galley boundries and stairway enclosures shall be self closing. PSCO observed fire screen door 15.1.04, 2.5.03 and 6.1.05 unable to close. 74SOLAS20 II-2/9.4.1.1.5.1

Manually operated call point complying with the FSS code...shall be readily accessible. PSCO observed manually operated call point 72 by the salty dog grill inoperable. 74SOLAS20 II-2/7.7

Fire extinguishers shall be situated...that their servicablity is not impaired by the weather vibrations or other external factors. PSCO observed a fire extinguisher by the deck safety work shop stowed without a bracket. 74 SOLAS20 II-2/10.3.2.4

Fire protection systems and fire fighting systems and appliances shall be maintained ready for use. PSCO observed a blocked fire extinguisher in store 17207.

The following functional requirements shall be met escape routes shall be maintained in a safe condition, clear of obstacles. PSCOs observed deck 5, crew training room emergency exit door failed to open. 74 SOLAS (20) II-2/13.1.2

The load line ring, lines and letters shall be painted white or yellow on a dark ground or in black on a light ground. They shall also be permanently marked. PSCOs observed STBD side deck line and load line mark were not contrasting color to the hull. ILLC Article 8

The administration shall satisfy itself what the general structure strength of the hull is sufficient for the drought corresponding to the free-board assigned. PSCOs were notified by a company representative that on 18Feb2024 the vessel made contact with pier 21 at Port Everglades causing a 5-10cm indentation along with some frame damage on the forward starboard side of the vessel bringing into question if the general structure strength of the hull is sufficient. ICLL 66/88 Annex I / Reg. 1(1)

Spaces are classified according to their fire risk shown in catagories (1) to (14). Life machinery space on deck 16, aft is a catagory (10) space where the storage of combustibles is prohibited. PSCO observed the storage of several combustibles. Code 16a, SOLAS 11-2/9.2.2.3.2.2

Safe escape routes shall be provided. In the bell box office, the audible alarm for the room-in-room arrangement did not give an immediate notification in the event of a fire. Code 16a, SOLAS II-2/13.1.1

Escape route signs... shall be of photoluminesent material or marked by lighting. In the crew training room, deck 5, zone 7, there were no exit signs for either means of escape. Code 16a, SOLAS II-2/13.3.2.5.1

A fixed fire-fighting system, required by this chapter shall comply with the provisions of the Fire Safety Systems Code. Each section of sprinklers shall include means for giving audible and visual alarm signal automatically at one or more indicating units whenever any spinkler comes into action. When testing section valves 36 & 54, flow switches did not activate on the bridge. FSS Code 8.2.5.2.1. Code 16a, SOLAS II-2/10.4.1.1

Escape doors from public spaces that are normally latched shall be fitted with a means of quick release. Such means shall consist of a door-latching mechanism incorporating a device that releases the latch upon the application of a force in the direction of escape flow. Primary means of escape from the gym was unable to be unlocked in the direction of escape IVO MCP 48. Cobe 16a, SOLAS II-2/13.3.2.6.2

For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in the catagories (1) to (14) below. NFPA Code 30 flammable liquid lockers were being used in the salon "chemical locker" and in the Horizon, stbd dishwashing area. The above mentioned lockers should only be used in catagory 11,12, or 14 spaces unless approved to EN11470-1 Type 60 or Type 90. Code 16a, SOLAS II-2/9.2.2.3.2.2.

Fire-fighting Systems and appliances shall be kept in good working order and readily available for immediate use. A fire station was blocked in club Fusion, deck 6, zone 7 and aft dining room deck 7, zone 7. In addition, in the Hearts and Minds chapel, chairs were blocking a fire hose station. Code 10c, SOLAS II-2/14.2.1.2

WHENEVER AN ACCIDENT OCCURS TO A SHIP OR A DEFECT IS DISCOVERED, EITHER OF WHICH AFFECTS THE SAFETY OF THE SHIP, THE MASTER OR OWNER OF THE SHIP SHALL REPORT AT THE EARLIEST OPPORTUNITY TO THE ADMIN, THE NOMINATED SURVEYOR OR RECOGNIZED ORGANIZATION RESPONSIBLE FOR ISSUING THE RELEVANT CERTIFICATE. IF THE SHIP IS IN A PORT OF ANOTHER CONTRACTING GOVERNMENT, THE MASTER SHALL ALSO REPORT IMMEDIATELY TO THE APPROPRIATE AUTHORITIES OF THE PORT STATE. VESSEL HAS EXPERIENCED AN ELECTRICAL FIRE ON LIFTING CRANE ABOVE #5 GENERATOR. VESSEL REP TO PROVIDE REPORT CONTAINING ROOT CAUSE OF SUCH FIRE, AND TO ENSURE PREVENTION OF SUCH FIRE IN ALL OTHER LIFTING CRANE INSTALLATIONS ABOVE OTHER SUCH GENERATORS. 74 SOLAS (20) I/11.C

The following functional requirements shall be met: escape routes shall be maintained in a safe condition, clear of obstacles. A laundry cart and a shelf of coveralls were observed IVO FSD 2.4.06 impeding the means of escape. In addition, IVO FSD 16.7.01, several pantry items were being stowed impeding the means of escape. Futhermore, on deck 4 amidships, IVO embarkation deck, plastic hotel service carts were being stowed in the passenger stair tower. CODE 10c, SOLAS II-2/13/1/2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed large inset and penetration of the hull on the port quarter. Damage is located above the waterline and is the result of the ship's allision with Piear 27 during mooring operations. 74 SOLAS (20) I/11(a) Code 60ac 02106

The emergency source of electrical power shall be capable, for a period of 36 hours: emergency lighting in alleyways, stairways, and exits. Emergency light located near the exit of the Skywalker Room was inoperable. 74 SOLAS 04 II-1/42.2.1.3

Openings in bulkheads and decks in passenger ships shall have means of securing them and shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions. PSCO discovered a B-15 fire door in crew laundry on deck 04 next to cabin P04101 and B-15 fire door on deck 03 of crew laundry did not properly close. 74 SOLAS 20 II-2/9.4

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Multiple installed ampules for hi-fog heads in spa treatment rooms had depleted fluid levels. 74 SOLAS (20) II-2/14.2.1.2

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirement shall be met: means shall be provided to control leaks of flammable liquids. #4 lube oil purifier had excessive leaks. 74 SOLAS (04) II-2/4.1.1

Emergency lighting for mustering and abandonment shall be tested at each abandon ship drill. The emergency lighting was not energized for the abandon ship drill. - 74 SOLAS (14) III/19.3.4.8

The Company should establish programs for drills and exercises to prepare for emergency actions. The company's procedures for abandon ship drills do not include energizing the emergency lighting. - 74 SOLAS (14) IX/3.1, ISM 8.2

During preparation and launching, the survival craft, launching appliance, and the area into which it is to be launched shall be adequately illuminated by lighting supplied from the emergency source of electrical power. 11 emergency lights are burnt out. - 74 SOLAS (04) III/16.7

Above the bulkhead deck there shall be at least two means of escape from each restricted space or group of spaces. Bosun locker has been subdivided with a locked gate dividing the primary and secondary escape routes. - 74 SOLAS (04) II-2/13.3.2.2

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire resistant divisions and protection of openings and penetrations in these division. Power-operated sliding fire door at starboard side deck 7 theater fails to open under power. - 74 SOLAS (14) II-2/14.2.1.1.1

74SOLAS(14) III/21.1.3 All survival craft required to provide for abandonment by the total number of persons on-board shall be capable of being launched with their full compement of persons and equipment within a period of 30 min from the time the abandon ship signal is given after all persons have been assembled, with life jackets donned. PSCOs unable to witness lowering and maneuvering of lifeboats due to bunkering operations. Lifeboat drills are to be conducted in next U.S. Port.

74SOLAS(04) II-1/45.1.3 All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCOs found two dead end wires in the deck 16 starbaord forward sanctuary.

02 - 0915 - In addition to the emergency lighting required by regulations II-1/41 and III/11.5, the means of escape, indlucing stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections.
