A fixed pressure-sensing device must be installed as close as practicable to the vessel vapor connection on a tank barge with a vapor collection system. The pressure-sensing device must measure the pressure vacuum in the main vapor collection line and have a pressure indicator located where the cargo transfer is controlled. Vessel did not have a fixed pressure-sensing device installed.

Lifebuoys must have the following attachments and fittings: at least one lifebuoy on each side of the vessel fitted with a buoyant lifeline that is of a breaking strength which is not less than 5 kiloNewtons (1,124 pounds-force). Vessel's forward lifebuoy lifeline and aft lifebuoy grabline were brittle and not of adequate breaking strength.

Vessel's 4P tank's closed gauging glass was cracked. Crew member installed new glass.
