Except as allowed in 33 CFR 151.33(a), no ship may carry a cargo of bulk liquid hazardous material or an NLS residue if the bulk liquid hazardous material or NLS is listed in Table 1 or carried under a written permission under paragraph (d) of this section unless the ship meets the following: The ship must have an IMO Certificate of Fitness issued under §153.12 that is endorsed to allow the cargo tank to carry the cargo if it is - - A United States non-self-propelled ship in the waters of another Administration signatory to MARPOL 73/78 and the cargo is a Category A, B, or C NLS. As specified, vessel may carry the cargoes mentioned above on domestic voyages ONLY until a Certificate of Fitness is obtained.

Install and operate a ballast water management system (BWMS) that has been approved by the Coast Guard. Vessel is restricted to the following options until a (BWMS) has been inspected and plan has been provided. - Use only water from a U.S. public water system (PWS), as defined in 40 CFR 141.2, that meets the requirements of 40 CFR parts 141 and 143 as ballast water. - Perform complete ballast water exchange in an area 200 nautical miles from any shore prior to discharging ballast water. - Do not discharge ballast water into waters of the United States. - Discharge to a facility onshore or to another vessel for purposes of treatment. 33 CFR 151.2025

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light. Equip one-half lifebuoys with self-igniting light. 46 CFR 199.70(a)(3)(ii)

Emergency instructions. Illustrations and instructions in English, and any other appropriate language as determined by the OCMI, must be posted. Post instructions on method of donning life jackets. 46 CFR 199.80(c)

The items contained in first-aid kit shall be as listed in Table 160.041–4(b). First-aid kit is not in compliance. 46 CFR 160.041-4(b)

Vessel's procedures for conditional occupancy failed to address all requirements in CG-CVC Policy Letter 16-04. Submit policy/procedures for safe passage of crew between towing vessel and barge to comply with conditional occupancy policy, and demonstrate deployment of safety netting or fall restraint system to the satisfaction of the attending Marine Inspector. Vessel is restricted from conditional occupancy until requirements are met. CG-CVC Policy Letter 16-04

#1 port ballast tank identified 2 locations of wastage. 1st and 3rd bay from forward bulkhead Gauge thickness of these areas in the presence of CG Marine Inspector and provide report.

Make permanent repairs to 10" fracture at frame 55.5

Repair/replace explosion proof fitting for heating cable on cargo deck. 46 CFR 111.105-9

Where exposed to normal wear and tear, repair plastic sheathing over heating cable on cargo deck. 46 CFR 111.105

3 inch crack in the #2 stbd socket on the JAK ladder. Make permanent repairs to the satisfaction of the attending Marine Inspector and ABS surveyor.

Permit to proceed to the closest repair facility and make permanent repairs to damaged area's listed on ABS report dated 4/28/15.

Dye pen all welds on both Port and STBD Pin Jacks and submit repair proposal for damaged areas.

Vessel failed to complete its annual inspection within the periods specified in 46CFR30.10-17. The owner representative informed Sector Puget Sound the vessel was late with its required annual inspection and scheduled the inspections on 08-09Jul2014, 8-9 days late from its due date (30Jun2014).

Starboard side pin hole leak near No.4 pin socket weld. Additional suspect areas noted in the No.2 and No. 5 socket locations.

Port side pin hole leak near Frame No.5.

Port side portential fractures at No. 2, 4 and 5 pin socket welds. Conduct NDT.

Make permanent repairs to cracked ballast pump containment combing at outside corner of port #5 cargo tank. Must be completed at next drydocking or when the cargo tank is gas freed for other purposes.

Maintain oil tight integrity in 5 starboard cargo tank. Submit repair proposal and complete satisfactory repairs prior to taking on cargo in the presence of the attending marine inspector.

Make permanent repairs to #5 port cargo tank as per submittal, utilizing NVIC 7-68 as guidance.

Make permanent repairs to fractured deck areas at base of port and stbd aft ladders.
