Certificates of inspection may be revoked or suspended by the Coast Guard where such process is authorized by law. This may occur if the vessel does not meet the requirements of law or regulations in this chapter or if there is a failure to maintain the safety requirements requisite to the issuance of a certificate of inspection. Vessel has sustained damage to hull rendering it unfit for service. The #2 void has water ingress to river level. Repair plan to be submitted to the cognizant OCMI prior to repairs. Vessel is permitted a one-time transit to Houston Shipyard Channelview for permanent repairs. An intermediate stop at Exxon Baytown and/or La Porte Fleet for cargo discharge is permitted.

Pressure vacuum valves shall be examined to determine that the operating mechanism is free and capable of activation. Marine Inspector discovered 05 pressure-vacuum valves missing identifying information regarding current testing. Test or replace each pressure-vacuum valve to the satisfaction of a Coast Guard marine inspector. Vessel is authorized a one time transit to the nearest facility to discharge cargo currently on board. After which, valves must be re-tested or replaced prior to taking on cargo. 46 CFR 151.04-5(h) 705C
