The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness of the vessel. Vessel experienced marine casualty that caused engine room to flood and rendered vessel machinery inoperable. Vessel must undergo full TPO Survey to be reviewed by Cognizant OCMI, vessel must undergo Drydock Exam, Internal Structure Exam, and COI inspection prior to returning to service.

Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. In areas where space limitations make deck rails impractical, such as at narrow catwalks in way of deckhouse sides, hand grabs may be substituted. The main deck of the vessel did not have hand rails or equivalent protection installed. Install hand rails or equivalent to the satisfaction of the USCG Marine Inspector.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The windings on port/stbd generator were saturated with oil presenting a fire hazard. Clean generator windings to the satisfaction of the USCG Marine Inspector prior to vsl movement.
