Towing vessels must be equipped with the following equipment, as applicable to the area of operation: Search light, controllable from the vessel's operating station and capable of illuminating objects at a distance of at least two times the length of the tow. Searchlights were damaged in bridge allision. Install searchlight to the satisfaction of the attending marine inspector. ****VESSEL IS RESTRICTED TO DAYTIME (SUNRISE TO SUNSET) AND FAIR WEATHER OPERATIONS ONLY**** This allowance ends at sunset on 23JUN2025 at which point a searchlight must be installed to continue operations and clear deficiency. Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel self-reported an allision with the Popps Ferry Bridge in Biloxi, MS, severely compromising all navigational equipment on the mast. Vessel is currently moored at the Fuel and Ice Dock in Biloxi, MS. Prior to any movement contact Sector Mobile for authorization. Conduct repairs to the satisfaction of the Coast Guard marine inspector. Cite: 46 CFR 143.205(a)

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light. Water light on one lifebuoy was inoperable.

A towing vessel must carry a combination of day and night visual distress signals indicated in Table 141.370 for specified areas where the vessel operates. Vessel was required 3 day and 3 night flares. Vessel had a total of 4 flares under approval series 160.121.

Flammable and combustible products must not be stored in machinery spaces, unless they are stored in a suitable container. Engine room had several full 5 gallon pails of unused oil as well as paint cans.

Nonconductive mats or grating must be provided on the deck in front of each switchboard. Several breaker panels had no mats or grating.

All portable fire extinguishers, semi-portable fire-extinguishing systems, and fixed fire-extinguishing systems required by this part must be approved by the Commandant (CG–ENG). Approval is valid only with approved bracket. Fire extinguisher located in forward hold was not mounted.

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. Pumps, one located at forward end of each main diesel engine, leaks lubricating oil onto hoses directly below the pumps; hydraulic steering system directional valves (located vertically on aft bulkhead in engine room) leaks hydraulic oil, and the port main diesel engine fuel supply line leaked fuel oil in vicinity of valve approximately a foot above deck plate.

No Certificate of Documentation issued under this part will be deemed valid for operation of the vessel until the vessel is marked in accordance with this subpart. Vessel not marked with vessel's name or hailing port.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. - Electrical distribution located on aft bulkhead of upper engine room missing blank - electrical junction box located on forward bulkhead of electronics room (below wheel house) had loose cover (Corrected) -Battery Boxes in electrical room (below wheel house) not secured to deck (Corrected)

Engine room to be fitted with one 40-B portable fire extinguisher for each 1,000 brake horsepower of the main engines or fraction thereof. Engine room had only 3 portable fire extinguishers meeting at least the 40-B rating. (Requirement also listed on COI.)(CORRECTED)

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Labels (2, one on each side of vessel) did not include sufficient purpose as crew was unable to identify whether or not activation secured all fuel or only fuel to specific machinery.

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. First aid kits were expired.

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Contrasting lettering was faded off.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Light covers missing or broken in the Mate's Bunkroom.

A fixed fire pump must be capable of: (1) Delivering water simultaneously from the two highest hydrants, or from both branches of the fitting if the highest hydrant has a Siamese fitting, at a pitot-tube pressure of at least 344(kPa) (50 pounds per square inch (psi)), and a flow rate of at least 300 liters per minute (lpm) (80 gallons per minute (gpm). Vessels fixed fire pump was unable to provide sufficient flow. portable fire pump placed on board until repairs could be made to the satisfaction of local Marine Inspector.

Each towing vessel must have either a self-priming, power-driven, fixed fire pump with 50 psi from the two highest hydrants, or a portable pump with 60 psi. Vessel's fixed pump unable to deliver sufficient pressure through two hoses; vessel not equipped with a fully outfitted portable fire pump.

Portable and semi-portable fire extinguishers must be inspected, maintained, and tested in accordance with the inspection, maintenance procedures, and hydrostatic pressure tests required by Chapters 7 and 8 of NFPA 10, Standard for Portable Fire Extinguishers, (ii) Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly checks not performed.

The whistle, bell and gong shall comply with the specifications in Annex III to these Rules (33 CFR part 86); Sound signal intensity and range of audibility require minimum for vessel length, 1 nautical mile. Vessel's wistle unable to meet required range.

Each lifebuoy with a floating electric water light must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the water light around the body of the lifebuoy. Lifebuoy's water light lanyard on port side of pilot house was less than 3 feet in length.

The sidelights of vessels of 20 meters or more in length shall be fitted with matt black inboard screens. A portion of the starboard sidelight screen had been painted white.

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. Superstructure bulkhead located on starboard side and just below the pilot house had 2 small holes which were not fitted with closure devices.

The Certificate of Financial Responsibility (COFR) Operator must apply for and ensure the vessel is covered at all times by a current COFR. IAW 33 CFR 138.170(b) During a period of non-compliance with COFR certificate requirements, ALL USE by the vessel of the navigable waters of the United States, of any port or other place subject to the jurisdiction of the United States is forbidden. In addition, IAW 33 CFR 138.170(e) any vessel subject to this COFR requirements which is found in the navigable waters without a COFR, or for which the necessary evidence of financial responsibility has not been established and maintained as required, is subject to seizure by, and forfeiture to, the United States. IAW 33 138.170(d) vessel is detained until the vessel is compliant with COFR requirements. COFR Currently Revoked.

Flammable liquid storage cabinets on 2nd deck do not meet specifications in stated cite and are constructed of aluminum. Replace storage cabinets with cabinets meeting specifications by next credit dry dock (due no later than July 13, 2025).

The frame or support of the engine room semi-portable fire extinguisher must be welded or otherwise permanently attached to a steel bulkhead or deck.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford protections against hazards to life, property, and environment. Vessel was involved in a marine casualty, grounding, and sustained damage do the hull. Vessel has hole in PORT generator room where water is ingressing. Make temporary repairs to hole/hull to the satisfaction of the marine inspector.

Survey damage, provide repair proposal, make permanent repairs to the satisfaction of the attending Coast Guard marine inspector.

Vessel must ensure flame bowls are properly installed on RACOR filters.

Vessel must have a 40-B fire extinguisher at the flammable materials storage cabinets.

All exits must be marked with phosporescent indiators to facilitate egress.
