All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weather-tight when closed. MI observed the gasket material in both port and starboard machinery houses to have gaps and degradation rendering them no longer weather-tight.

Activate a visible and audible alarm so that it can be seen and heard on the vessel where cargo transfer is controlled and in the cargo deck area; and activate an alarm early enough to allow the person in charge of transfer operations to stop the cargo transfer before the tank overflows. MI observed during testing of high and overfill alarms the audible alarm sounded approximately 15 seconds after the visual indication.
