01 The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to moving parts, hot surfaces and other hazards. Vessel reported that diesel fuel was discharged into the Delaware River during a bunkering operation due to a broken valve. The following is required: a report from the Recognized Organization (RO) detailing the cause of the spill, the corrective actions taken, and attesting to the full functionality of the associated equipment. The vessel may not conduct further bunkering operations.
