Valve operating rods (in cargo tanks) shall be of ample size, well guided and supported, and attached to the valve stems in a manner so as to prevent the operating rods from working loose.
Within the cargo tanks, approximately fifty percent of the bolts/nuts connecting reach rods to the piping closure devices were found to be loose. Had this condition progressed a reach rod may have lost connection to the closure device eliminating the tankerman's ability to stop the flow of cargo.
