Valve operating rods (in cargo tanks) shall be of ample size, well guided and supported, and attached to the valve stems in a manner so as to prevent the operating rods from working loose. Within the cargo tanks, approximately fifty percent of the bolts/nuts connecting reach rods to the piping closure devices were found to be loose. Had this condition progressed a reach rod may have lost connection to the closure device eliminating the tankerman's ability to stop the flow of cargo. 46CFR32.50-15(b)

Each non-rising stem valve lever operated valve, or other valve where because of design, the position of the disc or closure mechanism is not obvious must be fitted with an indicator to show weather the valve is opened or closed. Tank #1 block valve, tank #3 crossover block valve, tank #4 guard valve, tank #4 fill suction valve and #5 guard valve were missing valve position indicators. Provide valve position indicators for each valve listed.
