General arranagements plan shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations,...shall be kept up to date; and alterations thereto shall be recorded as soon as practicable. The location of the the fire lockers on the Fire Control Plan does not reflect the current location on board the vessel. Update fire control plan to reflect correct location on board vessel. 74 SOLAS (20)II-2/2.4.1

The company should ensure that each ship is manned with qualified, certificated, and medically-fit seafarers in accordance with national and international requirements. All three required Able Seamen in the Deck Department were endorsed as AB-Limited in violation of 46 U.S.C. 7312(b). Additionally, one of the three personnel in the Maintenance Department was not endorsed as an able seaman or as RFPNW as required by the COI in accordance with 46 CFR 15.515(a). The company is required to conduct an additional internal audit focused on resources and personnel, and to provide recommended corrective actions to prevent a future recurrence. 74 SOLAS (20) IX/3.1 / ISM Code 6.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Vessel reported the rescue boat lost the outboard motor while conducting a drill and does not have a spare on board. Provide report from recognized organization providing analysis of the cause of failure and statement regarding fitness for service. Report from RO is due prior to departure of the vessel. 74 SOLAS (20) III/20.2

Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. Waste storage receptacles on main deck aft/stbd side constructed of combustible materials (fiberglass). SOLAS 09 - II-2/4.4.2

The following cargo operations related items were identified and corrected: 1) Lightship characteristics increase from BWTS installation not incorporated in ballast/cargo loading program (ABS Houston letter dated 31 MAR 2020). C/M updated characteristics in program as per letter. 2) Cargo tanks 4P/1P/1S high level/overfill alarm tested intermittently or reversed. C/M adjusted test rods and inspected floats with MI. 3) ODME Annex II overboard valve position indicator failed during test. Indicator switch replaced/tested. 4) At cargo manifold: Benzene monitoring area and emergency shutdown stations needed proper markings. The following documentation/record keeping items identified and corrected: 1) Unattended engine room checkoff list used prior to unattended machinery space operations was not current version identified SMS; 2) Electrical distribution panel marked “P-P7 Panel X-FRMR and Receptacle For Arc Welder” in f'ocsle was found locked out. The panel was not tagged IAW the SMS Lock- Out/Tag-Out procedure and no log entry was created; 3) Records of bottom cleaning for UWILD and anchor chain cleaning not recorded IAW bio fouling management plan; 4) PSTP approval letter not found. The following electrical items identified and corrected: 1) Bonding straps on IG line forward of deck seal, EX-light at stbd/fwd end accom and #6S tank ullage radar; 2) Improper repairs to EX-light at forward/stbd side of cargo deck area (tagged-out for full repairs); 3) Battery charging cable tie to FFLB found with electrical tape/damaged sheathing. The following LSA items identified and corrected: 1) FFLB rations storage compartment securing latches found broken (3 of 4). 2) Gel coat damage found on rescue boat fwd/port gunnel exposing fiberglass.

Reported #4 cylinder piston ring cracked.

Cable penetrations in Class A boundaries have been compromised in multiple locations. Conduct survey of all penetrations, identify compromised transit runs, and seal appropriately.

Door leading to main engine space has been compromised with a penetration, steel bar and bolts running through the door. Replace door to approval configuration. Door is a Class A

Provide proper operation of LIRT/SAT-C.

Prove proper operation of main engine oil mist detector, verified by attending ABS surveyor, to the satisfaction of the U.S. Coast Guard.

Conduct a review of the vessel's PSTPs to ensure procedures match manufacturer's recommended practices. Submit all changes to USCG for approval & verification.

ODME inoperable, complete functionality tests of ODME prior to the operation of equipment to the satisfaction of the attending classification society surveyor and the attending marine inspector.
