Objective evidence from the TPO of damage survey and repairs to the satisfaction of the marine inspector shall be provided for review and approval prior to code 17 rescission. 46 CFR 143.205(c)

Received notification of marine casualty, vessel temporarily lost power due to a suspected faulty fuel pump solenoid on one of their 3 SSDG's. Power was restored via EDG, until the two other SSDG's could be brought online. Vessel may continue transit to Tampa for repairs. Repairs to be completed to satisfaction of attending TPO. Objective evidence attesting to the satisfactory repair IAW company TSMS must be submitted to issuing OCMI.

All Piping controls, valves, and alarms must be inspected; and the operation of controls, alarms, ventilation shutdowns and pressure operated dampers for each fixed fire-extinguishing system and must be tested to determine that the system is operating properly. MI identified two holes approximately 3/8" in diameter on bottom of fire main piping. Marine Inspector identified two holes approximately 3/8" in diameter. When system was charged system sprayed everywhere. 46 CFR 142.240(a)(5)

During inspection it was discovered that port & Stbd MDEs have exhaust leaks, due to cracks in the bellows. DPA was notified but failed to notify class of the deficiency, IAW OSG's Incident Reporting and Emergency Response procedures. Vessel is required to conduct an internal safety audit focusing on vessel maintenance and reporting incidents. Results of audit must be submitted to Class within 90 days.
