Except as provided by 46CFR15.725 of this part, no vessel may be navigated unless it has in its service and on board the crew compliment requied by the COI. Vessel sent a request to the MSU Toledo OCMI to sail short 01 Assistant Engineer. The request is temporary and the vessel is authorized to sail with this replacement until 24APR2026. Prior to the expiration of this deficiency the vessel/company shall provide proof that they are operating in accordance with the COI.

Immersion suits or anti-exposure suits must have the following attachments and fittings: (i) Each immersion suit or anti-exposure suit must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the immersion suit or anti-exposure suit. MI found immersion suit in ECR without a light. 46 CFR 199.70(c) 4(i)

A towing vessel that is certificated for limited coastwise, coastwise, oceans, or beyond 3 nautical miles from shore on the Great Lakes whose contract for construction was executed on or after August 27, 2003, except for those specified in paragraph (a)(3) of this section, must be equipped with both: (1) An approved 160-B semi-portable fire-extinguishing system to protect the engine room; and (2) A fixed fire-extinguishing system installed to protect the engine room. MI found the 160-B missing. 46 CFR 142.315(b)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. MI found F/O overflow pipe leaking onto deck from port generator. 46 CFR 143.205(a)

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. The equipment may be identified in words or with the appropriate symbol from IMO Resolution A.760(18) (incorporated by reference, see § 136.112 of this subchapter). MI found no lifejacket/survival suit storage markings in Pilot House and ECR. 46 CFR 141.340(h)

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. These devices must be suitable for the expected route. The Port side watertight door near the tank room no longer maintains watertight seal, renew with suitable gasket. 46 CFR 144.320(b)

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel self reported a small fracture in the STBD #2 BWT bottom shell plating. Vessel will have a temporary repair by installing a concrete box over the affected area. This temporary repair is subject to an annual inspection by a United States Coast Guard (USCG) Marine Inspector. A permanent repair, to the satisfaction of the local Officer in Charge, Marine Inspection (OCMI), must be completed at the vessel's next dry-docking.

The operator of a vessel with a capacity of 250 or more barrels of oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154 shall provide transfer procedures that meet the requirements of this part and part 156 of this chapter for transferring—(a) To or from the vessel; and(b) From tank to tank within the vessel. Tank to tank transfers are not detailed in the transfer procedures. Furthermore, tank level indicators are currently used to monitor levels of fuel tanks during transfer and bunkering operations. Inconsistent readings of the tank level indicators (TLI)in the fuel tanks is believed to have caused overflow from #2 port and starboard fuel tanks. Vessel rep states that all future bunkering and transfer operations will include manual soundings by the crew, not relying upon the TLI readings. Review transfer procedures and update to meet requirements of 33 CFR subpart C, to include newly implemented procedure to sound tanks manually during bunkering and transfer operations. Submit updated transfer procedures to MSU Duluth OCMI for review within 30 days.

Company/Vessel made a request to the OCMI at Sector Detroit to sail one QMED in lieu of one licensed Assistant Engineer for an initial period of four weeks. Master shall ensure work/rest requirements are met and all maintenance is conducted in accordance with company policy. Vessel is authorized to operate short one Asst. Engineer in accordance with proposal submitted for a period of 30 days, at which time the vessel must provide work/rest logs for the period of sailing short with a new request to the cognizant OCMI if no replacement is available. The vessel/company shall continue to make reasonable efforts to attain a replacement. Vessel shall ensure watches are conducted by licensed engineers and voyage length shall not be greater than from Toledo, OH to Silver Bay, WI. If no replacement is found, the company/master must provide a new request to the cognizant OCMI with: 1. Work rest logs for the previous period of crew shortage. 2. Attestation from the company that effort was made to fill the vacancy. 3. Statement from the Master and Chief engineer that the vessel is sufficiently manned to sail. 4. Coast Guard temporary manning request checklist 46 CFR 15.515 (a) 46 U.S.C. 8104

An existing vessel may be deemed by the OCMI to be in compliance with this subpart provided that: The vessel has been both in satisfactory service insofar as structural adequacy is concerned and does not cause the structure of the vessel to be questioned by either the [name withheld] concrete box repair effected to 6-10” hull fracture in #2 Port Ballast Tank, between frames 41 and 42, approximately 20 ft off centerline. Visually inspect, confirm structural integrity of the concrete box, and log condition every 3 months. Permanent repairs must be effected prior to end of next drydocking.

Vessels starboard rudder angle indicator is not functioning properly. Provide proof of proper operation of the steering system from all operating stations prior to transiting the St. Marys river.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions. Replace engine control room and starboard generation emergency lighting.

Propulsion machinery must be suitable for route and service. Presently the port MDE is inoperable due to faulty expansion joint for the turbo charger. Make proper repairs and prove proper operation of the port MDE prior to departure from Ashtabula, OH.

Vessel C/E discovered ruptured exhaust expansion joint on vessel's port MDE. Make permanent repairs to exhaust expansion joint and prove proper operation to satisfaction of attending USCG [name withheld] to be completed prior to departure from the port of Indiana Harbor, IN. During transit from Duluth, MN to Indiana Harbor, IN, notify the USCG Command Center of each cognizant OCMI as you prepare to enter and exit each zone.

A vertical fracture was found in the common bulkhead, number 4 ballast tank / number 3 port fuel tank, at frame 54. Effect temporary repairs as accepted by attending ABS Surveyor. Make permanent repairs at earliest dockside or drydock availability.

Make temporary repairs to the puncture in the starboard quarter IWO the steering gear compartment

Weight of check valve and addition of inline fuel meter caused flexing and vibration. These vibrations broke the threaded end of the flange as it entered the check valve "T" fitting.

Fuel oil control sensor failed in main engine electronic fuel control system, rendering port main diesel engine inoperable. Corrected and restored to normal operation.
