A cement box temporary hull repair was affected in the vessel's #7 starboard ballast tank. This temporary repair is considered satisfactory for continued vessel operations, but must be checked annually and permanently repaired by no later than the next scheduled drydocking to the satisfaction of USCG/ABS.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory. MSU Toledo MI and ABS surveyor were in attendance to verify permanent repairs on the 1P/2P BWT corrugated bulkhead for a leak that was found on a previous trip (ref MISLE activity #8229235). After hydro test, weld was found weeping on its inboard side right were the bracket was. Current repair is considered temporary and will need to be redone during winter layup 2026 no later than 15FEB2026.

This temporary repair is considered satisfactory for continued vessel operations, but must be checked annually and permanently repaired by no later than the next scheduled drydocking to the satisfaction of USCG/ABS.

In general, compliance with the standards established by the American Bureau of Shipping, see subpart 90.35 of this subchapter, will be considered as satisfactory evidence of the structural efficiency of the vessel. Vessel reported water ingress due to an object that is lodged in the hull of the forepeak, approx 4 frames to starboard from the centerline, at about the 16 ft draft mark. In accordance with proposed temporary repair plan, install cement box to satisfaction of class and provide class report to local OCMI, prior to departure of Silver Bay, MN. After arriving in Toledo, OH and installing proposed doubler over the patch on the exterior of the hull, provide class report attesting to temporary repair, to local OCMI in Toledo, OH. Conduct visual inspections on the cement box quarterly.

On 23JUN20 a tube in the cargo boom belt drive's hydraulic oil cooler failed causing oil to mix with the raw cooling water resulting in an overboard discharge. The heat exchanger is to be repaired and tested prior to placing the unit back into operation.

Repair and demonstrate proper function of starboard anchor windlass brake, prior to use of starboard windlass or deploying starboard anchor.

Paint all draft markings in color that is in contrast to the hull.

Stern lights inoperable.

Waterlight batteries expired.

Starboard lower navigation light inoperable.

Overhead light in anchor windlass space not properly secured.

Bow thruster generator machinery space, emergency fuel shutoff cable pull station is missing cover.

#1 generator reverse power relay inoperable.

Forward sidelights on barge (for towing behind) were damaged and missing. Normal operation is push ahead and proper lights are displayed illuminated on tug i.a.w. NavRuls.

Wireways located inboard of each fuel storage tank near overhead not made fire tight in way of bulkhead dividing workshop/fuel tank space to machinery room. 46 CFR 92.07-10(c)

Lube oil cooler for the generator that drives the conveyor system is leaking oil overboard. Do not use cooler until repairs have been effected.

Report of F/O line and jacket water system malfunction. # 1 SSG taken offline. Vessel using #2 SSG.

Provide a written proposal to address permanent repairs to be conducted to the hull failures resulting from the grounding of the vessel on it's earlier arrival to the loading dock at LaFarge North America Stone Quarry to the satisfaction of the [name withheld] Marie.

Permanent repairs to hull failures located in the port aft rake of the forepeak and the port foward bays of #1 port ballast tank are to be conducted to the satisfaction of an attending USCG Marine Inspector prior to the vessel resuming commercial operations.

Clean & provide mfg service rep report reflecting the Ludell Ultra-Efficient water heater was serviced, repaired, all safeties tested within limits, and is operating as designed by the manufacture. *Excessive fuel oil residue observed in unit’s water reservoir as required by 46 CFR 63.25-7

Submit Periodic Test Procedures for the Ludell Ultra-Efficient water heater; procedure must reflect step by step check for all alarms, safeties and or limit switches as required by 46 CFR 63.25-7(c)

Valve assembly was broken off lufting cylinder, releasing hydrolic oil on to deck. Repair underway, Removing sheard bolts and repair to valve assembly.

Clean all diesel engine battery boxes, line and or chock with wood strips as require by 46 CFR 111.15-5(d)&(f)
