The arrangements and position of openings in the cargo tank deck from which emission of flammable vapors can occur shall be such as to minimize the possibility of flammable vapors collecting in the vicinity of deck machinery and equipment which may constitute an ignition hazard. P/V (pressure vacuum) valves 25, 35, 45,65,75, 1P, 5P, 6P, 7P, 8P are all freely passing gasoline vapors on deck, from the vacuum side of the P/V valve. Tank pressures are at an average of 6.7 kPa. The vessel is restricted from conducting cargo operations.

Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such risks shall be taken to the satisfaction of the administration. Found all temperature probes serving cargo oil pumps 1-4 to have either damage to the cables where armoring was damaged, the gland inside the probe to be unserviceable where light was passing through, and dead end wires (2) not properly secured inside the enclosure. Top 1-4 have 3 probes each, which are located in the cargo pumproom. Vessel is restricted from conducting cargo operations.

The training manual shal contain instructions and information on the life saving appliances provided in the ship. Use of all survival equipment shall be explained in detail. Observed the absence of training for the hydrostatic releases in the training manual the installed hydrostatic releses did no mathc the other type of releases in the mnaunal.
