In tank vessels carrying Grade A, B, C, or D liquid cargo, cargo pumps shall be isolated from source of vapor ignition by gastight bulkheads. MI observed the a visible gap in the vapor seal in cargo pump 1 room. 46 CFR 32.60-20

Each hose must be marked with the maximum allowable working pressure. MI observed that the hot tar/asphalt hose did not have a clear indication of what the MAWP was marked on the hose. Correct prior to using the hose for hot tar/asphalt transfer operations. 33 CFR 155.800 33 CFR 154.500

Vessel currently has a heating cable, stapped to the ballast water piping, used for the heating under cold temperatures to prevent freezing. Unable to provide approval to be used in a hazardous location.

Ullage markings to indicate maximum level permitted for the cargo tanks are not permanent. Current lables are coming off and not meeting the regs.

Emergency Egress scuttle not being properly illuminated by the emergency lighting in the compartment.

Vessel currenly has portlights on cargo machinery spaces facing the cargo area. Currenly no backing plates are provided in the inside of the compartment.

Vessel unable to provide the laters cargo piping annual test.

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Vessel must implement an SMS procedure to properly check and log monthly fire fighting equipment inspections to the satisfaction of the Marine Inspector.

The owner, master, or person-in-charge of a tank vessel must ensure that portable and semi-portable extinguishers are inspected and maintained in accordance with NFPA 10. Vessel must replace the number 9 portable fire extinguisher bracket to the satisfaction of the Marine Inspector.

Each life jacket must have a life jacket light approved under approval series 161.112 securely attached to the front shoulder area of the life jacket. Vessel provided purchase order and delivery receipt of life jacket lights.

Each embarkation ladder must extend in a single length from the deck to the waterline with vessel in its lightest seagoing condition under unfavorable conditions of trim and with the vessel listed not less than 15 degrees either way. Marine Inspectors observed that the starboard side embarkation ladder when extended over the side from the deck to the its lowest ladder wrung was 8.3 feet above the waterline. The barge was in an even keel and in a ballasted condition. Vessel operator shall provide an embarkation ladder of suitable length for the starboard side life-raft embarkation station to the satisfaction of the Marine Inspector.

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port required to be marked on the stern of the vessel. Marine Inspectors observed 02-03 lifebuoys with adhesive stickers in various stages of peeling off of the lifebuoy, or not being legible due to abrasions on the stickers. Vessel operator shall inspect all 10 lifebuoys on the barge and ensure all lifebuoys are suitable marked with block capital letters with the name of the vessel and the name of the port to the satisfaction of the Marine Inspector.

Each item of lifesaving equipment required by this part to be carried on board the vessel must be approved. Marine Inspectors observed that the connection between the weak link assembly and the securing device used to keep the life-raft container secured to its cradle was a ratchet strap. Vessel operator explained that the manufacturer installed strap on the starboard life-raft broke during heavy seas and that it was replaced with a ratchet strap. The port side life-raft strap was subsequently removed and replaced with a ratchet strap as well. Vessel operator shall replace, or prove to the that the ratchet strap is an approved component for this lifesaving appliance.
