The engine room bilge alarm actuated all indicator lights at the operating station instead of the specific bilge location. Repair the bilge alarm indicators to show the specific location of alarms to the satisfaction of TVIB (TPO) or the Coast Guard.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. The wheeled semi-portable fire extinguisher was loose about the deck with no permanent support in place. Install permanent means of securing the semi-portable to the deck or bulkhead to the satisfaction of of TVIB (TPO) or the Coast Guard.

The crankshaft pulley, alternator pulley, and alternator drive belt for both the Port and Starboard Main Diesel Engines were exposed and found to have missing machinery guards, creating a safety hazard to the vessel crew. Rectify deficiency according to the satisfaction of TVIB (TPO) or the Coast Guard.
