Resolution: Technicians conducted trouble shooting and were unable to recreate the engine over heating. ABS cleared vessel, report is attached.

Vessel experienced a loss of propulsion in its Port MDE due to unknown causes. Prior to movement, determine root cause of shutdown and affect permanent repairs to the satisfaction of the TPO.

M/V SAVAGE VISION company policy (SOP 2105 SKIFF USE AND OPERATION 11.0) "SKIFF USE DURING NIGHTTIME IS STRICTLY PROHIBITED UNLESS IN AN EMERGENCY SITUATIOIN." The tug's skiff was used for a crew change, at night, on July 26, 2018, and was subsequently involved in an marine casualty sending the oncoming master to Lake Charles Memorial Hospital for injuries beyond first aid. Based on the clear violation of company policy by the master on watch, the off going relief captain and the on-coming relief captain there appears to be substantial non-compliance with the company's TSMS. Therefor, an external audit of the company's TSMS is required. In addition, it is recommended that crew change procedures be evaluated and implemented in the TSMS as necessary.
