Vessel is a harbor assist tug, and has not provided established procedures in the TSMS to respond to recovery of persons overboard while engaged in towing operations. Vessel has five days to rectify deficiency. - 46 CFR 140.425(a)

The fixed fire-extinguishing system is missing posted instructions at the position for manual operation, and a threaded press-button for manual activation per the original system design. The crew was unfamiliar with manual operation of the system. - 46 CFR 140.650
