The USCG was notified by the vessel rep that a crack was identified in the shell plating/hull structure. This fracture represents an immediate compromise to the vessel's watertight integrity. Vessel is effecting temp repairs and proceeding to drydock for permanent repairs. USCG to attend vessel in drydock to verify permanent repairs.

There shall be at least two means of escape from all general areas accessible to the passengers, if carried, or where the crew may be quartered or normally employed. At least one of these two means of escape shall be independent of watertight door. MI found the emergency escape scuttle for the FWD bow thruster room, sealed with a silicone-based compound around the entire diameter of the scuttle. Crew broke the silicone se by al and opened the scuttle. Master stated the gasket was replaced and he did not know why the compound was placed. Mi found an excessive amount of salt buildup in the engine room where the exhaust pipe to the Centerline Port engine discharges ovbd. This is an indication of a leak and a failed pipe or valve. The material, design, construction, workmanship, and arrangement of the main propulsion machinery, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. MI discovered a makeshift rubber shielding surrounding the inboard side of the flywheel on the Port Centerline propulsion engine. Chief Engineer stated it was placed there due to the rear main seal failing and leaking. Engines was noted as overdue for overhaul. Owner rep requested an extension of the deficiency correction until the vessel’s dry-dock in OCT 2024. Provide sufficient objective evidence and supplemental information that this failure can sustain operation until the requested timeframe. 46 CFR 58.05-1(a), action code 705, c, replace upon drydocking

Approved buoyant work vests are considered to be items of safety apparel and may be carried aboard vessels to be worn by crew members when working near or over the water under favorable working conditions. They shall be used under the supervision and control of designated ship's officers. When carried, such vests shall not be accepted in lieu of any portion of the required number of approved life preservers and shall not be substituted for the approved life preservers required to be worn during drills and emergencies. MI observed the crew using work vest when launching the rescue boat. Crew had no knowledge the vest could not be worn. Master was also wearing a self-inflating type lifejacket, however, there was no records of the vest maintenance IAW manufacturers specification ever being conducted. Vessel's COD incorrectly states the Hailing Port City as Franklin, LA, thus the Certificate of Inspection (COI) states the incorrect Hailing Port. Vessel shall obtain a new COD with the correct Hailing Port City of New Orleans, LA. Once a corrected COD is processed, a corrected COI will be issued.
