The vessel submitted a complete and accurate NOA on June 14, 2018 at 1502, approximately 13 hours before entering the Delaware Bay COTP Zone. This NOA did not meet the requirement in 33 CFR 160.212(a)(4)(ii)

Personnel Responsible for the operation of an incinerator installed in
accordance with the requirements of paragraph 6.1 of this regulation shall
be trained to implement the guidance provided in the manufacturers operation
manual. Crew members are putting metal into waste incinerator and door
assembly is damaged in addition to portions of interior refractory cracked
and broken...naritive.
