Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Vessel reported that while getting underway at the City dock, that the starboard main diesel engine (MDE) failed to go astern instead of going ahead and allied with a moored tug, GULF COAST (O.N. 653705)'s bow. Provide a causal analysis and a completion of repairs prior to conducting commercial operations to the satisfaction of the Third Party Organization (TPO) and the Coast Guard.
