While conducting annual inspection a pit was tested on the cargo tank top with inspector's pit gauge and found beyond 25% wastage. Several other areas on tank top show significant corrosion. Gauging is required to determine plate thickness in multiple areas.

The Scantlings, material, and workmanship, subdivision of cargo spaces and testing of tanks shall be at least equivalent to the American Bureau of Shipping (ABS) standards.
While moored in the Centerline fleeting area, Coast Guard marine inspectors observed an inset on T/B KOLACHE on the port side cargo expansion trunk, in the vicinity of #1 port cargo tank. The barge may operate, with the restriction that cargo may not be loaded in the #1 port cargo tank above the bottom plane of the expansion trunk. In no more than 30 days, the barge shall then proceed to a cleaning and repair facility. The damage is to be cropped out and renewed in kind. All repairs are to be completed to the satisfaction of an attending Coast Guard marine inspector.

During an internal structural examination the following damaged structural members were identified and must be repaired within 45 days.
1. A 2 ft section of a side chord and 2 ft section of transverse deck stiffener are both tripped in the port mid section of the bow.
2. A 1.5 ft section of a side chord in the #3 starboard void is crushed.
3. A 3 ft section of the floor frame in the #1 port void is crushed.
4. A 6 inch section of the side chord flange tore away from the web in the #2 port void.
