Machinery and electrical systems must be designed and maintained to provide for the safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. #1 Steering pump was reported as inoperable. The vessel's TPO has the option to clear this deficiency with notification to the local OCMI within 72-hours in accordance with CG-CVC Policy Letter 17-10.

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crewmembers are capable of performing the duties expected of them during emergencies. The portable fire pump must be self-priming and power-driven, with sufficient amount of lined commercial fire hose 15 meters (50 feet) in length, at least 40 mm (1.5 inches) in diameter and immediately available to attach to it so that a stream of water will reach any part of the vessel. The vessel is fitted with 75 ft. fire hoses and does not have a special consideration allowing the alternative arrangement.

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. There are no electrical outlets near the freezer in the forward hold and is powered from an extension cord. The extension cord is taught and poses a risk of coming loose.

Resolution: called out divers to make sure that there was no damage under the boat. Then quadrant and Jockey bar were taken to St Louis where the Machine Shop repaired the Quadrant by bending it back and re boring the holes. Then fabricated two new bushings.
