The high level indicating device must indicate visually the level of the liquid in the cargo tank when the liquid level is within a range of 1 meter (3.28 feet)of the top of the tank. #2 Cargo tank high level indicating device stick is inoperable.

Each flange connection face must have a permanent stud projecting outward that has a 12.7 mm diameter and is 1 inch long. It must be located at the top of the flange, midway between bolts, and in line with the bolthole pattern. Permanent stud missing on vapor flange. 46 CFR 39.2001(j)

The vessel was found to be operating without a valid COI. Vessel is prohibited from operating until it obtains a new COI.
