Where practicable and except as provided in 144.515, each space where crew may be quarted or normally employed must have at least two means of escape. Vessel's forward escape hatch was seized.

"The master, owner, operator, agent, or person in charge of a vessel bound for a port or place in the United States, unless specifically exempted by § 151.2015 of this subpart, must ensure the maintenance of written or digital records that include the information required to be reported by § 151.2060 of this subpart and the sediment information in paragraph (a)(1) of this section." "Information on ballast water tanks that are to be discharged into the waters of the United States or to a reception facility. Include the following for each tank discharged:(i) The numerical designation, type and capacity of the ballast tank.(ii) The source of the ballast water. This includes date(s), location(s), and volume(s). If a tank has undergone ballast water exchange, provide the loading port of the ballast water that was discharged during the exchange.(iii) The date(s), starting location(s), ending location(s), volume(s), and method(s) of ballast water management.(iv) The date(s), location(s), and volume(s) of any ballast water discharged into the waters of the United States or to a reception facility" Ballast water records onboard the vessel do not show any location where the vessel discharged ballast water. Company shall ensure that this vessel and all others in the fleet comply with ballast water recordkeeping requirements, specifically ensuring the documentation of: "The date(s), location(s), and volume(s) of any ballast water discharged into the waters of the United States or to a reception facility"

A towing vessel must be equipped with a fixed fire-extinguishing system installed to protect the engine room. Fixed fire-extinguishing system was in unserviceable condition due to deteriorated supply hoses, corroded control heads and corrosion on the pressure shut down switch. Servicing company needs to re-certify the system IAW 46 CFR 142.240 and repairs are to be verified by a Marine Inspector.

Alarms must be visible and audible at the operating station. Audible high bilge level alarm did not sound at the operating station.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. 2 lifebuoys on the starboard side and 1 on the port side were in an unserviceable condition.

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: all bilges are kept free from accumulation of flammable materials. Inspector observed lube oil accumulating in the bilges.

Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Inspector observed inadequate emergency lighting in the forward escape and forward machinery space. Additional emergency lights need to be added. In addition, emergency lights in the pilothouse stairwell and machinery space need bulb replacement.

Towing machinery used to connect the towing vessel to the tow must be suitable for its intended service. Significant corrosion and deterioration was noted on the assist winch between the main drum and brake rotor. Vessel must submit a plan of repairs prior to operating the winch.
