Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure all bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. An excessive amount of oil was noted in the engine room bilge spaces.

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. The painter line for the vessel's life raft was found to be incorrectly connected.

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. Starboard main requires crew protection from rotating machinery IVO clutch, aft of the main engine.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Newly installed port generator had electrical connections that were exposed with electrical tape. Connections must be made in a junction box designed for use in machinery space.

Amendments made to a Vessel Security Plan that are approved by Marine Safety Center may be initiated by The Coast Guard upon a determination that an amendment is needed to maintain the vessel's security. The company owner or operator will have at least 60 days to submit its proposed amendments. Until amendments are approved, the company owner or operator shall ensure temporary security measures are implemented to the satisfaction of the Coast Guard. The vessel submitted temporary security measures to the satisfaction of the cognizant OCMI. The vessel to submit necessary amendments to the VSP for approval within 60 days. 33 CFR 104.415(a)(1)(ii)

Vessel owners and operators must ensure that their vessels are operating in compliance of 33 CFR 104. MI observed the following items on-board the vessel: Master's non-familiarity with the vessel security plan(VSP) regarding vessel's stowaways on-board, CSO's non-familiarity with required notification as per 33 CFR 101.305, and an absence of access control information within VSP related to vessel transiting through sensitive areas. Vessel to conduct an external audit of the vessel's VSP within 60 days, to the satisfaction of the cognizant OCMI. 33 CFR 104.115(a)

MI observed the following items on-board the vessel: Master's non-familiarity with the vessel security plan(VSP) regarding vessel's stowaways on-board, CSO's non-familiarity with required notification as per 33 CFR 101.305, and an absence of access control information within VSP related to vessel transiting through sensitive areas. Vessel to conduct an external audit, to the satisfaction of the cognizant OCMI, regarding the VSP to address the following non-conformities prior to vessel's departure from Sector San [name withheld] zone. 33 CFR 104.115(a)

46 CFR 143.205 Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Port generator is not operational due to being disconnected for ongoing maintenance.

46 CFR 140.515 ll crewmembers must be provided with health and safety information and training that includes: (1) Content and procedures of the owner or managing operator's health and safety plan; (2) Procedures for reporting unsafe conditions; (3) Proper selection and use of PPE appropriate to the vessel operation; (4) Safe use of equipment including deck machinery, rigging, welding and cutting, hand tools, ladders, and abrasive wheel machinery found onboard the vessel; (5) Hazard communication and cargo knowledge; (6) Safe use and storage of hazardous materials and chemicals; (7) Confined space entry; (8) Respiratory protection; and (9) Lockout/Tagout procedures. No evidence was found that vessel crew has been trained and no training records were found.

46 CFR 140.915 The following list of items must be recorded in the TVR, or official logbook: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. No evidence was found that the above-required were being conducted, nor logged in a logbook, and no entries were found in the "HELM" software installed at the operating station.

46 CFR 143.205 Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. A portable diesel generator was installed on the 02 starboard aft deck without submitting plans or approval from the local [name withheld] detailed plans and specifications to the local OCMI for review. 705C - Prior to 11/1/2024.

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan's implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. A BMW plan was not provided on board.

All crewmembers must be provided with health and safety information and training. Crewmember training on the health and safety plan has not been completed.

The owner or managing operator must implement procedures to identify and mitigate the listed health and safety hazards. A complete health and safety plan was not available on board.

The name of the towing vessel must be marked or painted in clearly legible letters on each EPIRB. No marking or NOAA sticker was present on the EPIRB.

Machinery must be maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Pressure gauge located on forward winch requires replacement.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Breaker panel in galley missing cover over breaker #16.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. Containers of Flammable product found stored on open shelves located on main deck next to laundry space. Requires removal or appropriate storage in a compartment free of ignition sources.

A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. The current medical certificate for the Master expired April 8, 2020 and is no longer valid.

Marine employers shall establish programs for the chemical testing for dangerous drugs on a random basis of crewmembers on inspected vessels. The current crewmembers on board were not listed on the company's Maritime Consortium certificate.

Towing vessels must be equipped with an illuminated magnetic compass. The light on the magnetic compass was not operational.
