To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover.
Starboard fuel shut off valve is incorrectly configured. The valve handle is missing a stop and when pulled beyond the closed position, continues to move 180 deg. to the open position.
Renew valve handle and configure the valve to allow operation from the open position, to the closed position, and no further.

Exhaust lagging for all operating engines is unsatisfactory. Lagging wrap is not continuous and exposes several sections of hot exhaust piping.
Re-wrap exhaust lagging to cover all exposed areas and secure lagging wrap with proper banding to prevent future exposure.

Hose is serving an submersible pump that is no longer in use. Make effective repairs to ensure watertight integrity of the void.

Vessel is equipped with remote fuel valves at the port and starboard holding tank. However to the unique design of vessel's fuel system, using the inline "stripping" tank, closing the remote fuel valves serving the holding tank would not stop the fuel supply to the propulsion engines or generators in a timely manor. Either, install remote fuel shut off valves in the fuel supply line serving each propulsion engine and generator or reconfigure fuel piping system to exclude "stripping" tank.

Propulsion and generator low lube oil pressure and high cooling water temperature, high bilge level, low hydraulic steering fluid level. Additionally alarm setpoints must be tested twice within every 5 years, not to exceed 3 years in accordance with table 46 CFR 143.245. Vessel crew could not test engine alarms. Vessel's engine alarm system does not meet configuration requirements listed in 46 CFR 143.230. Install alarm panel meeting requirements and have alarms tested by marine inspector.

Deck in crew head is wasted to the point that it has holes in it. Condition of deck is a safety hazard to the crew. Vessel is authorized to conduct temporary repair using epoxy.
