Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. The 2 refrigerators located in the pantry were plugged into a multi-outlet adapter, which was then connected to an extension cord to reach the nearest outlet. Remove the "daisy-chain" configuration and provide an alternative.

A Cargo Ship Safety Radio Certificate shall be issued after an initial or renewal survey to a cargo ship of 300 gross tonnage and upwards which complies with the relevant requirements of chapter IV and any other relevant requirements of the present regulations. No CSSRC was issued to the vessel. The certificate shall be available prior to operating in international waters.

No IEEC was issued to the vessel. MARPOL (2017) Annex VI Reg 6.4

Any ship of 400 gross tonnage and above but less than 10,000 gross tonnage shall be fitted with oil filtering equipment complying with paragraph 6 of MARPOL Annex I, reg 14. Vessel is restricted from going on an international voyage until the vessel crew can demonstrate the proper operation of the installed OWS to the attending CG Marine Inspector.

Resolution: Vessel rep conducted permanent repairs and an operational test int he presence of the attending CG Marine Inspectors.
