Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. PSTP Test 5.11 for DG Safety Shutdown-High Jacket Water Temp details a shutdown at 225 degrees F after 10 seconds. Conducting test as described resulted in shutdown after 40 seconds. SSDGs established set points identify a 226 degrees F set point and 30 second time delay to shutdown. 46 CFR 61.40-10(a)

General arrangement plans shall be permanently exhibited for the guidances of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions, and fire extinguishing appliances. The Fire Control Plan lists numerous watertight doors as being self-closing but are incapable of self-closing due to the design of the door lacking a spring latch and receiving strike plate as well as the watertight dogs preventing full closure. Additionally, AR-AFF bucket for forward E/R was not located in correct location on fire control plan or placarded. SOLAS 74 Ch. II-2 Reg. 15.2.4
