A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Vessel crew discovered water in the port side void from an unidentified source which impacts the watertight integrity of the vessel. Prior to movement, vessel must make permanent repairs to the satisfaction of USCG Marine Inspectors. While moored IVO Carpenter's Bayou, the vessels' crew observed the vessel listing on the stern; further noticed the vessel taking on water in the stern void. vessel is allowed to transit to the Shipyard to affect permanent repairs to the satisfaction of the USCG Marine Inspector.

While shifting within the ACBL Fleet, ITV DAVID M. FLEMING experienced a Loss in Propulsion due to damage to the starboard shaft propeller. The vessel managed to maneuver back to dock; make notifications, conduct diver survey to determine damage. Vessel allowed to transit to Southwest Shipyard with assist tug. Prior to cargo operations, vessel must conduct permanent repairs to the satisfaction of Third party Organization (TVIB) or attending CG Marine Inspector.

During damage survey at shipyard while vessel was in dry dock, marine inspectors found a 5 inch crack in the starboard aft bilge knuckle that caused the internal frame to fracture. Crop and renew bilge knuckle and internal frame to satisfaction of USCG Marine Inspector.
