A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. Inspector witnessed a fitting sheer off the port shaft seal causing water to enter the engine room. Crew quickly stopped the flooding and are sourcing a new replacement fitting.

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Inspector noted that the armored cable throughout the engine room was supported by metal banding without chafe gear and causing chafing of the protective layer. There are also numerous cables supported on pipes and other frames throughout the engine room without chafe gear.

A towing vessel must be maintained and operated so the watertight integrity and stability of the vessel are not compromised. DMI received a report that the vessel grounded while underway and has in operable rudders. Develop a corrective action plan to determine full extent of damage to the vessel, and effect permanent repairs to the satisfaction of the vessels TSMS and attending Marine Inspectors.
