The condition of the ship and its equipment shall be maintained to conform to with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or person on board. Oxygen and nitrogen bottles on the aft mooring deck were not properly stored off the deck.

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II- I/42 or II-1/43. PSCO observed non-operational emergency lighting forward of lifeboat #16 and #18.

All doors shall be capable of remote and automatic release from a continuously manned central control station, either simultaneously or in groups, and also individually from a position at both sides of the door. Indication must be provided at the fire control panel in the continuously manned central control station whether each of the remote-controlled doors are closed. The release mechanism shall be so designed that the door will automatically close in the event of disruption of the control system or central power supply. Release switches shall have an on off function to prevent automatic resetting of the system. Hold-back hooks not subject to central control station release are prohibited. PSCO observed galley fire door 3.14 on deck 10 not capable of properly latching.

Port State Control Officer responded to a casualty onboard resulting from undetermined processes within the Safety Management System regarding the opening of a confined space and determining the root cause analysis. The engine shall be capable of operating for not less than 5 min after starting from cold with the lifeboat out of the water. FPVE observed a hydraulic fluid leak on lifeboat #14 rendering the steering gear inoperable. 74SOLAS(20)III/34 LSA Code 4.4.6.3

Fire detection and fire alarm systems, shall be kept in good order so as to ensure their required performance if a fire occurs. PSCO observed three inoperable smoke detectors in the Lido Galley on deck 9 and one inoperable smoke detector in engine casing room on deck 6. 74 SOLAS 20 11-2/14.2.1.1.2 16ac

All life saving appliances and arrangements shall comply with the applicable requirements of the code. The engine shall be capable of operating for not less than 5 minutes after starting from cold with the lifeboat out of the water. Doors and door frames in "B" class division and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the division. PSCO observed FSD 2.02 on deck A and FSD 5.12 on deck 2 unable to close. 74 SOLAS 97 11-2/31.1 16ac

Fire protection systems and firefighting systems and appliance shall be maintained and ready for use. PSCO observed fire dampers disconnected from vents (5618.4.226) in space HK07 and inoperable when reconnected. 74 SOLAS 20 1-2/14.1.1 16ac

The international load line certificate shall not be delivered to the ship until the officer or surveyor acting under the provisions of Article 13 of the present convention have certified that the marks are correctly and permanently indicated on the ships side. PSCO observed that the load line markings on the ship did not match the markings on the International Load Line Certificate. ILLC/Reg. 9 50ac

The Load Line ring lines and letters shall be painted white or yellow on a dark background or in black on a light background. PSCO observed the deck line painted white against a white background. ILLC/ Reg. 8 50ac

Fire protection systems and firefighting systems and appliances shall be maintained ready for use. PSCO observed obstructed sprinkler head on deck A by FSD2.17. 74 SOLAS 20 11-2/14.1.1 10ac

Fire doors in main vertical zone bulkhead and stairway enclosures designed to reopen upon contacting an object in it's path sail reopen sufficiently to allow a clear passage of at least .75m but not more than 1m. PSCO observed inoperable safety bar on deck 2 FSD 3.03, deck 1 FSD 3.05 deck 5 FSD 3.05, and FSD 5.12. 74 SOLAS 97 11-2/30.4.2 10ac

Electrical installations shall be such that electrical services for safety will be ensured under various emergency conditions. PSCO observed unsecured junction box on tender #6. 74 SOLAS 04 11-1/40.1.2 10ac

The provisions on the prevention of accidents and the protection of health and employment which may be applicable to the work of seafarers, and shall specify measures for the prevention of accidents which are peculiar to maritime employment. Fire doors in main vertical zone bulkheads and stairway enclosures shall be self-closing and shall have an approximately uniform rate of closure of no more than 40 s and no less than 10 s. PSCO observed single leaf door on deck 6, 7.06, in the stairway not latching. 74 SOLAS (97) II-2/30.4.1

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Team Leader on lifeboats 11, 15, and 17 were unfamiliar with how to operate Emergency Steering on lifeboats. 74 SOLAS (20) III/19.2.1

Doorways and corridors and intermediate landings included in means of escape shall be sized in the same manner as stairways. PSCO observed the emergency escape corridor in the spa to be obstructed with a linen cart and trashcan leaving less than 900 mm in clear width. 74 SOLAS (97) II-2/28.1.5.3
