Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. MI notified by SCC-SF of the vessels loss of propulsion for starboard engine. Provide plans for repair to the satisfaction of MI prior to movement. 46 CFR 143.205(a)

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency. Station bill did not have all required emergency duties. 46 CFR 140.405(b)

The master must ensure that all lifesaving equipment is properly maintained and ready for use at all times. Life saving equipment on main deck berthing areas were improperly labeled. 46 CFR 141.230

Each towing vessel must have a reliable means to provide notification when an emergency condition exists or an essential system develops problems that require attention. High water alarms for engine room were not labeled at the operating station. 46 CFR 143.230(a)

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page... Transfer procedures were not IAW with 33 CFR 155.750. 33 CFR 155.750

On a merchant vessel of the United States the construction of which began after March 4th, 1915..(5) each crew berthing area shall be equipped with information regarding-(A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (B) procedures and resources to report crimes, including sexual assault and sexual harassment, including information-(i) on the telephone number, website address, and email address for reporting allegations of sexual assault and sexual harassment to the Coast Guard;(ii) on vessel owner or company procedures to report violations of company policy and access resources; (iii) on resources provided by outside organizations such as sexual assault hotlines and counseling;(iv) on the retention period for surveillance video recording after an incident of sexual harassment or sexual assault is reported; and (v) additional items specified in regulations issued by, and at the discretion of, the Secretary of the department in which the Coast Guard is operating. (d) In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage. A vessel at anchor... of less than 50 meters in length may exhibit an all-round white light where it can best be seen. Vessel's anchor light was inoperable. Repair to the satisfaction of the MI. 46 CFR 83.30 (b)

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. Vessel's emergency lighting in the pilothouse was inoperable. Repair to the satisfaction of the MI. 46 CFR 143.410 (b)

Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Electrical panel in machinery space was loose. Repair to the satisfaction of the MI. 46 CFR 143.400 (a)

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. 03 watertight doors were not labeled om both sides. Repaired in the presence of the marine inspector. 46 CFR 144.160 (e)

Stowage positions for lifejackets stowed in a berthing space or stateroom and all lifejacket containers must be marked in block capital letters and numbers with the minimum quantity, identity, and, if sizes other than adult or universal sizes are used on the vessel, the size of the lifejackets stowed inside the container. The equipment may be identified in words or with the appropriate symbol from IMO Resolution A.760(18). Vessel did not have accurate quantities labeled in the berthing areas. Repaired in the presence of the marine inspector. 46 CFR 141.340 (h)

Each lifebuoy on a towing vessel must: Be marked in block capital letters with the name of the vessel. 01 lifebuoy onboard had faded markings. Repaired in the presence of the marine inspector. 46 CFR 141.360 (b)(4)

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage, including: Charts, including electronic charts acceptable to the Coast Guard, of appropriate scale to make safe navigation possible. The vessel had the recreational version of the Rose Point Navigation System on board instead of the commercial version.

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must: Have suitable propulsion controls to provide the operator full control at each operating station. The STBD main engine bridge shutdown was inoperable from the operating station.

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. The line throwing appliance must have: (1) The equipment on the list provided by the manufacturer with the approved appliance; and (2) An auxiliary line that: (i) Is at least 450 meters (1,500 feet) long; (ii) Has a breaking strength of at least 40 kilonewtons (9,000 pounds-force) and (iii) Is, if synthetic, of a dark color or certified by the manufacturer to be resistant to deterioration from ultraviolet light. The vessel had an incomplete line throwing appliance (1 of 4 required rocket units) and no auxiliary line.

Every towing vessel equipped with an inflatable survival craft must have, in conspicuous places near each inflatable survival craft, approved placards or other posted instructions for launching and inflating inflatable survival craft. The vessel was missing posted life raft instructions.

Any towing vessel with alternating watches (shift work) or overnight accommodations must identify the duties and duty stations of each person onboard during an emergency ,including: (1) Responding to... flooding; (3) Launching survival craft; (4) Taking action during heavy weather; (6) Taking action relative to the tow; (7) Taking action in the event of failure of propulsion, steering, or control system. The emergency duties and duty stations required by this section must be posted at each operating station and in a conspicuous location in a space commonly visited by crew members. The on board station bill found was missing the referenced emergency scenarios. 46 CFR 43.205 (b) & (c)

A record of the type, size, and service of each towline, face wire, and spring line, used to make the towing vessel fast to her tow, must be available to the Coast Guard or third-party auditor for review. The following minimum information is required in the record: The dates when examinations were performed, the identification of each item of towing gear examined, and the name(s) of the person(s) conducting the examinations. Inspection records found missing required information.

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page: (2) A description of each transfer system on the vessel including: (i) A line diagram of the vessel's transfer piping, including the location of each valve, pump, control device, vent, and overflow. Vessels transfer procedures found without description of transfer system.

Each operator or agent of a vessel with a capacity of 250 or more barrels of fuel oil ... shall designate, either by name or by position in the crew, the person in charge (PIC) of each transfer to or from the vessel and of each tank-cleaning. No list of designated PIC’s was presented for review.

The transfer procedures required by § 155.720 must be: (a) Available for inspection by the COTP or OCMI whenever the vessel is in operation; (c) Permanently posted or available at a place where the procedures can be easily seen and used by members of the crew when engaged in transfer operations. Inspectors were presented two different versions of transfer procedures, one printed and one electronic. Vessel to make single, correct version available for use by the crew.

A vessel that is exposed to salt water more than 6 months in any 12-month period since the last examination or initial certification must undergo a drydock and internal structural examination at least twice every 5 years, with not more than 36 months between examinations. The vessel has not conducted a drydock and internal structural examination within the allotted time frame. 46 CFR 137.300(a)(1)

Each vessel must be designed and constructed to minimize fire hazards insofar as reasonable and practicable. Both Port and Stbd fwd Fuel tanks have plastic tubular gauge. Replace plastic tubular gauge with glass tubular gauge (sight glass).

Each vessel must be designed and constructed to minimize fire hazards insofar as reasonable and practicable. Bulkhead insulation in the fidley leading to the galley needs to be repaired or renewed to the satisfaction of the OCMI.

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. The space above the accommodation space/below the operating stations needs to be labeled inside and outside to show exit. 46 CFR 144.160(f)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Install line throwing device on board vessel.

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Semi-portable fire extinguisher was removed from the vessel. 46 CFR 142.230(c)

Propulsion machinery, including main engines, reduction gears, shafting, bearings, and electrical equipment and systems, must be maintained to ensure proper operation. Hydraulic hose fitting above the port MDE is leaking. Vessel's rep tightened loose fitting. Operational test satisfactory. 46 CFR 143.205(c)(1)

Each watertight door and watertight hatch must be marked on both sides: "WATERTIGHT DOOR--KEEP CLOSED" or "WATERTIGHT HATCH--KEEP CLOSED." Vessel's watertight hatches missing required stenciling. 46 CFR 144.160(e)

Each towing vessel must have a reliable means to provide notification when an emergency condition exists, with alarm provided for the day tank's low fuel level. Day tank alarm labeled for high fuel level instead of low fuel level. 46 CFR 143.230(a)(7)

Electrical systems must be designed and maintained to provide for safety of the persons onboard. Overhead lights discovered missing guards/globes in the following locations: Main deck head; Stbd aft area of the engineroom; PFD storage room, and; Upper deck bunkroom at the end of the passageway. 46 CFR 143.205(a)

Each vessel must carry a certificate with a valid endorsement while subject to the Bridge-to-Bridge Act. Vessel missing required FCC Bridge-to-Bridge Certificate. 47 CFR 80.1005

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. Port and stbd air receivers in the engineroom contained painted over nameplates, making the MAWP and serial numbers unreadable. 46 CFR 143.300(c)

Electrical panels or other enclosures containing more than one source of power must be fitted with a sign warning persons of this condition and identifying where to secure all sources. Main electrical panel in the engineroom missing required signage noting where to secure power sources. 46 CFR 143.250(b)

Electrical components that pose an electrical hazard must be in an enclosure. Noted numerous dead-end wires located under the pilothouse console. 46 CFR 143.400(g)

ZERO LENGTH OR BEAM: no dimension information (AIS Antenna Position ABCD values equals 0)
