Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. Marine inspector observed open electrical junction boxes in engine room. Install covers on electrical junction boxes. 46 CFR 143.400 (a)

Each towing vessel must perform Test of Alarm setpoints at a frequency of Twice every 5 years. Marine inspector observed the vessel did not have a test procedure in place to test operation of the main engine high cooling water temperature. Develop test/ record procedure of testing high coolant water temperature alarm and all other alarm set points. 46 CFR 143.245 (b)

Each towing vessel must perform Test of pressure vessel relief valves at a frequency of Twice every 5 years. Vessel could not provide last testing of pressure relief valves for the pressure vessels. Install new or test certified pressure relief valves. 46 CFR 143.245 (b)

Each towing vessel equipped with rudder(s) must have a means to monitor and control the position of the rudder(s) at each operating station. Marine inspector observed the rudder angle indicator was inoperable. Repair rudder angle indicator must be repaired to proper operation. 46 CFR 143.225 (b)

Each remote valve control must be marked in clearly legible letters, at least 25 millimeters (1 inch) high, indicating the purpose of the valve and the way to operate it. Marine inspector observed the remote valve control for the fuel shutoffs were illegible. Re-apply markings per the CFR. 46 CFR 143.260 (e)

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessels VHF did not have MMSI or GPS programmed in. Program in the MMSI and GPS for DSC. 46 CFR 140.715 (a)

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. MI found pressure relief valves on both the pressure vessel and compressor had not been tested since 2020. 46 CFR 143.245(a)

All fire-suppression and detection equipment and systems on board a towing vessel must be maintained in accordance with the attached nameplate, manufacturer's approved design manual, or as otherwise provided in any TSMS applicable to the vessel. MI observed that 160-B was expired. 46 CFR 142.240(b)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. MI observed that the overboard discharge was not secured.

All training required in this section must be documented in owner or managing operator's records. Owner did not have record of health and safety refresher training. 46 CFR 140.515(f)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel had dead end wire in the upper machinery space. Rectify within 7 days. 46 CFR 143.205

All machinery and equipment that is not in proper working order (including missing or malfunctioning guards or safety devices) must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. MI observed an AED that had not been serviced or trained with that was not tagged out. 46 CFR 140.505(c)

Each towing vessel certificated under the provisions of this subchapter must be in full compliance with the terms of the COI.

All vessel equipment must be used in accordance with the manufacturer's recommended practice and in a manner that minimizes risk of injury or death. This includes machinery, deck machinery, towing gear, ladders, embarkation devices, cranes, portable tools, and safety equipment. MI observed that TVR had a section for the aft skiff to be inspected, which had not been done for several years. The skiff either needs to be inspected IAW the TVR or removed from the vessel. 46 CFR 140.505

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.655; (9) Record of inspection of towing gear, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. None of the items to be recorded were found onboard the vessel or completed by the new owner. 46 CFR 140.915(a)

A towing vessel must carry adequate and up-to-date charts, maps, and nautical publications for the intended voyage. The vessel does not have charts for Tampa Bay onboard. 46 CFR 140.705

A towing vessel of 26 feet or more in length and more than 600 horsepower, engaged in commercial service must have on board a properly installed, operational Coast Guard type-approved AIS device. The AIS device found onboard was not transmitting the vessel's position to the other AIS devices. 33 CFR 164.46

Each towing vessel must be fitted with a general alarm that complies with 46 CFR 143.235. The general alarm was found inoperable when tested. 46 CFR 143.235

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The port and starboard generators were not operational during the inspection. The operator will prove proper operation of the generators prior to receiving a new COI. 46 CFR 143.205

Survival craft must be stowed in accordance with the requirements of § 199.130 of this chapter, as far as is practicable on existing towing vessels. The operator elected to use a combination of survival craft for the 8 total persons permitted onboard. A 4 person skiff that meets requirements in § 141.330(a) through (f) is onboard and a 4 person SOLAS B life raft is onboard. The life raft is not mounted and the hydro static release is not connected to the unit. 46 CFR 141.310

Each towing vessel must have either a self-priming, power-driven, fixed fire pump, a fire main, and hoses and nozzles in accordance with paragraphs (a) through (d) of this section; or a portable pump, and hoses and nozzles, in accordance with paragraphs (e) and (f) of this section. The fire pump was not self priming and could not be energized from outside of the space due to the sea water pick-up valve being closed. The valve handle was missing and the valve could not be opened without the use of additional tools. 46 CFR 142.325

All seals and gaskets must be properly maintained to prevent leaks of flammable or combustible liquid, as those terms are defined in 46 CFR subpart 30.10, into the machinery space. The flexible hose fitting on the starboard main engine was found leaking diesel fuel when in operation. 46 CFR 143.220

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. The two aft ventilation fans for the engine space do not have closing devices that seal the opening in case of fire. 46 CFR 144.605

All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable. The engine room bilge was found with approximately 1 foot of oily bilge water throughout and the MI was unable to witness a test of the bilge pumping system without creating a pollution incident. The operator shall remove all oily bilge water and dispose of it IAW all applicable laws. The operator shall test the bilge pump in the presence of the attending MI once the bilge is clean. 46 CFR 142.220

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space. The emergency lighting in the engine room is not sufficient and does not illuminate the egress path out of the engine room. 46 CFR 143.410

A 40-B portable fire extinguisher must be located near the storage room or cabinet for storage of flammable or combustible products. The vessel is outfitted with two flammable storage cabinets which contain gasoline and flammable paint. An additional 40-B portable extinguisher is required for the second storage cabinet. 46 CFR 142.225

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. The starboard pressure vessel data plate is not legible due to excessive paint. 46 CFR 143.300

Closure devices must be provided for deckhouse or hull penetrations, which open to the exterior of the vessel and which may allow water to enter the vessel. Electrical penetrations in the engine room aft bulkhead on the main deck were found unsealed and provide access for water to enter the engine room space. 46 CFR 144.320

Prior to assuming duties related to the safe operation of a towing vessel, each crewmember must receive training to ensure that they are familiar with: Watchstanding terms and definitions; Duties of a lookout; Communication with other watchstanders; Change of watch procedures; Procedures for reporting other vessels or objects; and Watchstanding safety. Crewmember training must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. The new crewmembers onboard did not complete the navigation safety training prior to assuming duties onboard. The training is not logged. 46 CFR 140.645

The control panel and operating station alarm panel displayed a trouble alarm and could not be tested. 46 CFR 27.201
