The relief valves for the pressure vessel's were set to 200 PSI which is the MAWP, not ensuring that the pressure does not climb above the MAWP by more than 10 percent. 46 CFR 143.300(a)

The master of a towing vessel must ensure that drills are conducted and instructions are given to ensure that all crew members are capable of preforming the duties expected of them during emergencies. During the first attempt of a fire drill, the crew was not able to demonstrate a fire drill that followed on board emergency procedures. For a fire in the engine room, the crew did not utilize on board extinguishers or effectively communicate with the bridge. A second drill was run successfully. 46 CFR 140.420(a)

Resolution: Spoke with Captain about why first drill was failed. Ensured crew knew how to use extinguishers and close all fire dampners. Witnessed crew run second sucessful drill.

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. 01 detector was missing from accommodation space, and 01 was missing batteries. Examine each tail shaft for bends, crack, and damage . . . Vessel had excessive vibration from a bent wheel. The vessel will come out of the water for repairs.

Resolution: Both of the smoke detectors in the accommodation spaces were properly installed with the correct batteries.
