Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure the following: (a)All bilges and void spaces are kept free from accumulation of combustible and flammable materials and liquids insofar as practicable.
MIs witnessed approximately 6-8 inches of fuel/oil/water mixture throughout the E/R bilge. Deck plates had pooled fuel/oil in numerous locations.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions.
Numerous leaks were found on the main engines producing more than a drip per minute. The crank case breather vents installed on both M/Es are constructed of PVC.
The bilge pump has exposed wiring on the back side of the motor.
There are numerous loose, dead end, and spliced wires in the console located in the wheelhouse.
MIs witnessed excessive play and shuddering of the steering rudders while conducting a steering test.
