Smoke detection in berthing spaces. Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces. This may be accomplished by an installed detection system, or by using individual battery-operated detectors meeting UL 217. Detection systems or individual detectors must be kept operational at all times when the crew is on board the towing vessel. Three berthing spaces smoke detectors were missing. Provide the missing smoke detectors to the satisfaction of the attending Marine Inspector. 46 CFR 142.330(b)

Each towing vessel must be operated in compliance with; (1) Applicable sections of the Federal Water Pollution Control Act, including section 311 of the Federal Water Pollution Control Act, as amended (33 U.S.C. 1321). The vessel has pneumatic double diaphragm pump permanently plumbed into the bilge manifold system that permanently ties into the ballast water overboard valve. Decommission this pumping arrangement and abandon the pipe where it ties into the ballast water overboard discharge to the satisfaction of the attending Marine Inspector. 46 CFR 140.655(a)(1)

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Pressure relief valve testing for the pressure vessels is overdue. 1) Provide a valid test record within three years of the last test; or 2) Test in the presence of the attending Marine Inspector. 46 CFR 143.235(b)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited by the Environmental Protection Agency under 40 CFR 140.3 or 140.4, the operator must secure each Type I or Type II device in a manner which prevents discharge of treated or untreated sewage. Acceptable methods of securing the device include (1) Closing the seacock and removing the handle; (2) Padlocking the seacock in the closed position; (3) Using a non-releasable wire-tie to hold the seacock in the closed position; or (4) Locking the door to the space enclosing the toilets with a padlock or door handle key lock. Vessel sewage overboard valve is not secured. Secure using one of the methods above to the satisfaction of the attending Marine Inspector. 33 CFR 159.7

An exposed hazard such as gears and rotating machinery must be protected by a cover, guard or rail. This is not meant to restrict access to towing equipment such as winches, drums, towing gear or steering compartment equipment necessary for the operation of the vessel. Forward port anchor winch coupling machinery guard missing. Install the guard to the satisfaction of the attending Marine Inspector. 46 CFR 144.820

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110. 1) The two ring buoy lights on the port and stbd bridgewings were inoperable, correct lights. 2) The retroreflective tape on the stbd and port bridge wing and the port and stbd 02 deck was wasted; renew the retroreflective tape. 3) The stbd 02 deck ring life buoy was not in a serviceable condition; replace. All to be resolved to the satisfaction of the attending Marine Inspector. 46 CFR 141.360

Towing vessels must meet the communications requirements of 33 CFR part 26 and 33 CFR 164.72, as applicable. Vessel Bridge-to-Bridge certificate expired. Provide a valid Bridge-to-Bridge certificate to the satisfaction of the attending Marine Inspector. 46 CFR 140.715

Internal combustion engine exhaust ducts, galley exhaust ducts and similar ignition sources must be insulated with noncombustible insulation if less than 450 mm (18 inches) away from combustible material. Installations in accordance with ABYC P-1 or NFPA 302 will be considered as meeting the requirements of this section. There is wooden board int port exhaust stack that is less than 18 inches away from an uninsulated engine exhaust duct. 1) Replace board with non-combustible material; 2) Insulate the engine exhaust duct; or 3) Remove the board; to the satisfaction of the attending Marine Inspector. 46 CFR 144.415

No person may operate an oceangoing ship of 100 gross tons and above but less than 400 gross tons that is fitted with main or auxiliary machinery spaces unless the ship has a means on the weather deck near the discharge outlet to stop each pump that is used to discharge oily mixtures. Missing oily mixtures fixed piping system discharge to a reception facility pump stop on the weather deck. Provide the stop controller for the pump to the satisfaction of the attending Marine Inspector. 33 CFR 155.420(5)

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page: A description of each transfer system on the vessel including: A line diagram of the vessel's transfer piping, including the location of each valve, pump, control device, vent, and overflow. Missing the required transfer system line diagram. Provide the fuel transfer diagram to the satisfaction of the attending Marine Inspector. 33 CFR 155.750(a)(2)(i)

No person may transfer oil or hazardous material to or from a vessel unless each person in charge, designated under §§ 154.710 and 155.700 of this chapter, has filled out and signed the declaration of inspection form described in paragraph (c) of this section. Missing the Declaration of inspection (DOI) for pre-trip to Hawaii and July 7, 2025, fuel transfers. Provide the missing DOIs to the satisfaction of the attending Marine Inspector. 33 CFR 156.150(a)

Safety orientations and other crew training must be documented in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. No safety orientation records could be provided. Provide the required safety orientation record to the satisfaction of the attending Marine Inspector. 46 CFR 140.410

Crewmember training required by this section must be conducted as soon as practicable, but not later than 5 days after employment. No health and safety information and training records could be provided for the new hires working on the vessel. Provide the required 5-day records to the satisfaction of the attending Marine Inspector. 46 CFR 140.515(c)

The master, owner, operator, agent, or person in charge of a vessel equipped with ballast tanks that operates in the waters of the United States must employ one of the following ballast water management methods: Use only water from a U.S. public water system (PWS), as defined in 40 CFR 141.2, that meets the requirements of 40 CFR parts 141 and 143 as ballast water. Vessels using water from a PWS as ballast must maintain a record of which PWS they received the water from as well as a receipt, invoice, or other documentation from the PWS indicating that water came from that system. Furthermore, they must certify that they have met the conditions in paragraphs (a)(2)(i) or (ii) of this section, as applicable, and describe in the BWM plan the procedures to be used to ensure compliance with those conditions and thereafter document such compliance in the BW record book. Vessels using water from a PWS must use such water exclusively unless the usage is in accordance with § 151.2040 of this subpart. Vessels using PWS water as ballast must have either— Previously cleaned the ballast tanks (including removing all residual sediments) and not subsequently introduced ambient water; or never introduced ambient water to those tanks and supply lines. Vessel uses only water from a U.S. public water source and was unable to provide records of which PWS they received the water from as well as a receipt, invoice, or other documentation from the PWS indicating that water came from that system. Provide the required PWS records to the satisfaction of the attending Marine Inspector. 33 CFR 151.2025(a)(2)

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under approval series 160.040. Additional equipment: The line throwing appliance must have an auxiliary line that is at least 450 meters (1,500 feet) long. Missing the required auxiliary line. Provide the auxiliary line to the satisfaction of the attending Marine Inspector. 46 CFR 141.385(b)(2)

Except as provided in paragraph (a)(4) of this section, each towing vessel operating north of lat. 32° N. or south of lat. 32° S. must carry the number of immersion suits. Missing immersion suits approved 36-month service interval documentation from and approved manufacturer servicing facility. Provide servicing documentation to the satisfaction of the attending Marine Inspector. 46 CFR 141.350(a)

Refresher training must be repeated annually and may be conducted over time in modules covering specific topics. No health and safety information, annual training records could be provided. Provide the required health and safety records to the satisfaction of the attending Marine Inspector. 46 CFR 140.515(d)

Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. Alarm set points as prescribed by 46 CFR 143.230 are due and no record of readiness and testing could be provided. Provide the required test record documentation to the satisfaction of the attending Marine Inspector. 46 CFR 143.245

The results of the inspections required by 33 CFR 164.76 must be documented in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel. Missing towline and terminal gear for towing vessel alongside and pushing ahead minimum information as prescribed by 46 CFR 140.820(b). Provide the required minimum information prescribed by 33 CFR 164.74 and .76 to the satisfaction of the attending Marine Inspector.

Inspection will ensure that the vessel is in satisfactory condition and fit for the service for which it is intended. Marine Inspector observed that the Load Line was expired and is out of compliance for route. Complete Load Line Certificate requirements, be issued certificate from ABS and have observed by Coast Guard marine inspector.

Each towing vessel must perform the applicable tests for engine alarm set points twice every five years with no more than three years elapsing since the last test. Alarms were overdue for required testing. provide evidence of testing. 33 CFR 143.245 (b)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons on board under normal and emergency conditions. Port rudder post leaking excessively. Correct/adjust to prevent water intrusion into the stern lazarette. 33 CFR 143.205 (a)

Each towing vessel must be fitted with a general alarm that: Has installed, in the engine room and any other area where background noise makes a general alarm hard to hear, a supplemental flashing red light that is identified with a sign that reads: “Attention General Alarm—When Alarm Sounds or Flashes Go to Your Station”. Vessel did not have a flashing red light installed in the engine room. Install a flashing red light in the engine room which activates with the general alarm. 46CFR143.235(a)(3)

A towing vessel of 26 feet or more in length and more than 600 horsepower, engaged in commercial service must have on board a properly installed, operational Coast Guard type-approved AIS Class A device. AIS broadcast information does not match registered information. Correct information to match. 33CFR164.46(b)(ii)

A 40-B portable fire extinguisher must be located near the flammable products storage room or cabinet. No portable extinguisher was near the cabinet. Mount a fire extinguisher in a location near the cabinet. 46CFR142.225(d)

Each towing vessel operating north of lat. 32° N. or south of lat. 32° S. must carry at least one appropriate-size immersion suit, approved under approval series 160.171, for each person on board. Immersion suits were not available for inspection as they were being serviced ashore. Make available immersion suits for inspection prior to operating outside of the Columbia River. 46CFR141.350

Each towing vessel operating on oceans, coastwise, limited coastwise, or beyond 3 nautical miles from shore upon the Great Lakes must carry a Category 1, 406 MHz satellite Emergency Position Indicating Radio Beacon (EPIRB) that meets the requirements of 47 CFR part 80. The EPIRB must be stowed in its float-free bracket with the controls set for automatic activation and be mounted in a manner so that it will float free if the towing vessel sinks. The hydrostatic release for the EPIRB was expired. Install new hydrostatic release for the float free arrangement prior to operating outside of the Columbia River. 46CFR141.380

11128-Line-throwing Appliance-Line throwing device must meet approval series 160.040. Appliance found on board is the old rifle type device and does not meet approval. 46 CFR 141.385. Vessel given 30 days to aquire replacement.

08105-Engineer's Alarms-Vessel must have low lube oil and high temperature alrms on the main engines and generators. Vessel's crew were not equipped to test the low lube oil pressure and high cooling water temp. alarms on the vessel's main engines and generators. Inspectors verified alarms were installed. Vessel given 1 year to provide tech report to the CG attesting to the proper function of alarms. Verified with CID prior to issuing extended resolution date. 46 CFR 143.230.
