Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Vessel experienced loss of RPM from Port generator with unknown cause. Vessel is to transit to Mobile bay sea buoy to affect repairs. Notify CG upon completion of repairs.

Essential systems or equipment must be regularly tested and examined. Tests and examinations must verify that the system or equipment functions as designed. If a component is found unsatisfactory, it must be repaired or replaced. Vessel reported to USCG that Port Main Diesel Engine suffered from a high pressure crank case condition. This resulted in a high pressure alarm and lifting of explosion covers as evident by oil on deck plates. Chief engineer ordered reduction of speed and shut down of MDE. Prove proper operation of port MDE prior to departure.

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company, investigated and analyzed with the objective of improving safety. While USCG marine inspectors were investigating causality to port MDE stated above the following was noted: 1) No record was made in logbook regarding the incident as required by OPPM 6-03. 2) No record of check list being used for Main Engine Failure as required using form OPPM-3-02. 3) No objective evidence JSA was generated and approval was not granted to remove port MDE out of service as required by OPPM-1-09 Critical Equipment. 4) No objective evidence repair request was entered in the Operations Management System as required by OPPM-1-09. 5) No objective evidence Notification Report was generated as required by sec 6-03 using form # SMS-430. An internal audit, the results of which to be reported to ABS, is required to be conducted with focus on the procedures for reporting defects to the company and the proper use of checklist developed by the company.
