PSCE's observed two separate indicators, 1 in the starboard steering gear flat and the other in the forward eng. room, that failed to give a visual indication upon activation of the general alarm.

In the vessel's new generator room, observed several areas where openings and penetrations are not maintaining the A-60 boundary in accordance with their fire safety control plan. Both emergency exit doors are missing parts to the handle/lock. The forward door has the wrong handle/lock which allows the cut-out to be exposed. The forward door has two cut-outs allowing wires/cables to pass through.
