Pressure vessels over 5 cubic feet in volume and over 15 lbs/square inch must be equipped with one or more spring-loaded relief valves to prevent pressure from exceeding the MAWP by more than 10%. Existing relief valve was last tested Jan of 2023. Install new pressure relief valve not exceeding 200psi (MAWP) by more than 10% for the vessel's #1 receiver. Or, as an alternative, test valve, ensure proper operation, and document test.

A towing vessel with overnight accommodations and alternating watches. . . must have a system to detect when its master or mate (pilot) becomes incapacitated. Vessel's crew stated the system was operational, however they could not test the system to prove proper operation. Provide documentation on board the vessel outlining the steps necessary to test the system. Prove proper operation and ensure these tests are conducted and documented as required by vessel's TSMS and 46 CFR 143.245(b). Vessel may operate for 14 days provided there is an extra person provided in the pilothouse.

Vessel's port main engine suffered a fire on or about the turbocharger. Repair port main engine and any damaged wiring and lighting in the engine room to the satisfaction of the TPO and CG Marine Inspector.

To stop the flow of fuel in the event of a fire or break in the fuel line a remote fuel shut off valve must be fitted on any fuel line that supplies fuel directly to the propulsion engine. The port and starboard emergency fuel shut off valves for both main engines did not secure the fuel when actuated during an engine room fire. Repair and test to the satisfaction of the TPO and CG Marine Inspector.

Towing vessels of more than 65 feet in length must carry at least the minimum number of hand portable fire extinguishers set forth in Table 142.230(d)(2). Vessel crew used 3+ portable fire extinguishers to fight an engine room fire. Recharge or replace the extinguishers as required by the vessel's COI to the satisfaction of the TPO and CG Marine Inspector.
