Before the ship leaves port and at all times during the voyage all life saving appliances shall be in working order and ready for immediate use. 02 life buoys were found to be missing the required reflective tape. One of the life rings was also found with a non-operational light. Crew has replaced reflective tape and light on life buoys. 74 SOLAS III/20.2

Fire hoses shall be of material approved by the administration and sufficient in length to project a jet of water to any of the spaces in which they may be reuqired. 01 fire hose was found in the engine room with a torn liner. Crew replaced fire hose with spare onboard the vessel. 74 SOLAS II-2/5(f)

After any survey of the ship, no change shall be made without the sanction of the Administration. The jacket cooling water system for the main engine is not operating as intended. When the main engine is not in operation, the jacket water is actively leaking from cylinders causing the crew to run flexible hoses from the cylinders to the bilge. 74 SOLAS I/11(a)

Heavy exhaust discharge from the main engine within the engine room was witnessed by PSC team during ignition and transition between ahead and astern propulsion. Both PSCOs' 4-gas detectors alarmed when entering the vicinity of the main engine. Approximately 40% of the E/R overhead space is covered in exhaust residue. Crew noted that this has been a reoccurring issue for the past 2 months. Required that the vessel supply a marine chemist to gas free the space to ensure that the space is safe for work. 74 SOLAS I/11(a)

Locations of oily bilge water include underneath the shaft, port side of the main engine beneath a leaking drain pipe, and throughout the area around the lube oil purifier section of the engine room. Specifically, the area around the shaf was approximately 3 feet deep with oily water. The crew attempted to remove the majority of oil from the bilge. 74 SOLAS I/11(a)

Fire fighter's outfits shall comply with the Fire Safety Systems Code. Fire Fighter outfits had hood protection of neck, face, and shoulders that were not afixed to helmet and were not donned for fire drill. Fire fighter's neck was exposed.
