Efficient guard rail or bulwarks shall be fitted to all exposed parts of freeboard and superstructure decks. Topside guard rail on starboard side is broken and does not provide adequate protection for crew. ICLL 66 Annex I/25

Before the ship leaves port and at all times during the voyage all life saving appliances shall be in working order and ready for immediate use. 02 life buoys were found to be missing the required reflective tape. One of the life rings was also found with a non-operational light. Crew has replaced reflective tape and light on life buoys. 74 SOLAS III/20.2

Fire hoses shall be of material approved by the administration and sufficient in length to project a jet of water to any of the spaces in which they may be reuqired. 01 fire hose was found in the engine room with a torn liner. Crew replaced fire hose with spare onboard the vessel. 74 SOLAS II-2/5(f)

After any survey of the ship, no change shall be made without the sanction of the Administration. The jacket cooling water system for the main engine is not operating as intended. When the main engine is not in operation, the jacket water is actively leaking from cylinders causing the crew to run flexible hoses from the cylinders to the bilge. 74 SOLAS I/11(a)

Heavy exhaust discharge from the main engine within the engine room was witnessed by PSC team during ignition and transition between ahead and astern propulsion. Both PSCOs' 4-gas detectors alarmed when entering the vicinity of the main engine. Approximately 40% of the E/R overhead space is covered in exhaust residue. Crew noted that this has been a reoccurring issue for the past 2 months. Required that the vessel supply a marine chemist to gas free the space to ensure that the space is safe for work. 74 SOLAS I/11(a)

Locations of oily bilge water include underneath the shaft, port side of the main engine beneath a leaking drain pipe, and throughout the area around the lube oil purifier section of the engine room. Specifically, the area around the shaf was approximately 3 feet deep with oily water. The crew attempted to remove the majority of oil from the bilge. 74 SOLAS I/11(a)

Fire fighter's outfits shall comply with the Fire Safety Systems Code. Fire Fighter outfits had hood protection of neck, face, and shoulders that were not afixed to helmet and were not donned for fire drill. Fire fighter's neck was exposed.

1099 Any required automatic sprinkler and fire alarm and fire detections system shall be capable of immediate operation at all times. The fire detection system has numerious issues with the smoke and heat detectors, relocated detectors, detectors without face covers. 40A 74 Solas II-2 Reg 12(A)(i)

0981 A ship which undergoes repairs, alterations, modifications and outfitting shall continue to comply with at least the requirements previously applicable to the ship. Side shell of the vessel in the engine room starboard side second deck, logitudional frame are currently under repair, and no longer supporting the side shell. Repairs are being conducted under class. 17AC ICLL Annex I art. 10

While transiting outbound COTP Baltimore, [name withheld] reported a loss of propulsion. Vessel was diverted outside of the navigable channel and dropped anchor. Class surveyor (LR) was issued prior to departure deficiency.

[name withheld] reported the vessel's S band radar was not functioning properly while on the outbound transit from COTP Baltimore. SecBalt issued prior to departure deficiency to attest to the proper functionality of the S band radar.

05 Food rations are wasted and need to be replaced. Provide receipt of new food rations to USCG prior to departure.

SOLAS 74 CH.1 REG.11 A soft patch is located on the fire line above the #1 saltwater cooling pump. The line supplies a fire hydrant on the second deck of the engine room. Class to attest to permanent repairs of the fire line.

Master & 3/O (both req'd by Minimum Safe Manning) lacked Flag STCW endorsements/receipts of application. Vessel did not meet staffing req'ts of Minimum Safe Manning document.

Oil filtering equipment/bilge piping: petcock valve fitted on overboard discharge pipe outboard of the 3-way valve & Oil Content Meter. Valve is not on any piping diagrams; see narrative for more information.

Engine Room: 4 pumps lacked guards for rotating machinery (#1 & #2 jacket water pumps / #1 & #2 central cooling pumps).

Man the vessel in accordance with the safe manning certificate. The required second engineer's license expired 21JUN06.

Make repairs to the pin hole leak in the inert gas line (branch) serving 4S cargo tank. Inert gas pressure is adequate. Make temporary repairs to the IG line before departing St Croix and to the sat of class.

Calibrate and prove proper operation of the oxygen and IG pressure charts. Both devices indicate pressure however do not record.
