Second Engineer Officer, unless the person serving holds an appropriate, valid STCW endorsement issued in accordance with part 11 of this subchapter. On ships designed for special purposes, such as automobile or railroad car ferries, where installation of such bulkheads would defeat the purpose for which the ship is intended, equivalent means for controlling and limiting a fire shall be substituted and specifically approved by the Administration. During Inspection, Marine Inspector found that cargo fan damper 3706 and emergency fire pump space damper on focsle failed to operate correctly. 60 SOLAS II/37(d)

The main inlet and outlets of all ventilation systems shall be capable of being closed from outside the space in the event of a fire. The accommodation exhaust vent closures located on the port and starboard side of the 08 deck found with silicone installed on the knife edge. 60 SOLAS II/47(a)

The training manual... shall contain instructions and information, in easily understood terms illustrated whenever possible, on the lifesaving appliances provided in the ship and on the best methods of survival. The following shall be explained in detail... recovery of survival craft... including stowage and securing. The training manual found on-board refers to Rottmer-type releasing gear in the section covering the recovery of survival craft. 74 SOLAS (20 cons) III/35.3.13

The company shall ensure an internal audit is conducted and corrective action taken within three months focusing on the noted discrepancies.

Cargo ports and other similar openings in the sides of ships below the freeboard deck shall be fitted with doors so designed as to ensure he watertightness and structural integrity commensurate with the surrounding shell plating. The Chief Engineer found as the only signing authority in the Oil Record Book. The SMS does not define and document the various authorities granted to the Chief Mate and Chief Engineer while in Reduced Operating Status without a Master on-board the vessel.
