The certificate of inspection (COI) issued by an Officer in Charge, Marine Inspection (OCMI), to a vessel required to be inspected under 46 U.S.C. 3301 specifies the minimum complement of officers and crew necessary for the safe operation of the vessel. Vessel was not fully manned in accordance with COI. Rectify prior to activation. 46 CFR 15.501, 705c

Vessel is required 2 tug escorts greater than or equal to 5,000 aggregate total horsepower (one tug must have a minimum of 2,000 horsepower); or 2 tug escorts greater than or equal to 50 ton bollard pull to assist the vessel to the sea buoy upon departure. Repair starboard aft window penetration on bridge; currently has "direct tv" cable running through.

Replace (04) expired primer ejector cartridges for line throwing appliance.

Repair watertight door leading to flying bridge, conduct chalk test to ensure tightness.

Repair numerous missing and broken dogs on bridge windows and ensure hingepins are properly seated and equipped with cotter pins.

Renew rotted storm railing along port and starboard bridgewings.

Repair broken dog on stairtower entrance to weatherdeck: 34'-6' flat on boat deck.

Free frozen stancions on boat deck in way of accommodation ladders along port and starboard side.
