FORT INDEPENDENCE

An emergency switch must be provided in the normally ungrounded main supply conductor from a battery. The switch must be accessible and located as close to the battery as practicable. Port battery switch suffered a catastrophic failure and caught fire. Replace switch prior to carriage of passengers.

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by table 1 to § 118.500(b). Crew discharged 03 portable extinguishers during a fire casualty. Replace the extinguishers prior to carriage of passengers.

All machinery must be maintained in such a manner as to afford adequate protection against machinery failure. While operating, the vessel experienced a loss of propulsion on the starboard engine. Affect repairs to the satisfaction of the attending marin inspector.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, and other persons, and the vessel, from electrical hazards, including fire, caused by or originating in electrical equipment. The starboard generator had oil pooling and dust accumulation around the air supply intake adjacent to the windings. Clean both the starboard and port generator windings and casings of oil and debris to the satisfaction of the attending marine inspector. 46 CFR 120.200(b)

Electrical equipment on a vessel must be installed and maintained to protect... from electrical hazards, including fire. The port aft light fixture was deemed unsafe due to the fixture smoking upon installation of light bulb. Repair lighting fixture to the satisfaction of the attending marine inspector. 46 CFR 120.200(b)

Waste receptacles must be constructed of non-combustible materials with no openings in the sides or bottom. The port side bathrooms had three combustible trash receptacles. Replace trash receptacles to the satisfaction of the attending marine inspector. 46 CFR 116.405(i)

Each window... must be capable of withstanding the maximum load from wave and wind conditions expected. The port window aft of the passenger embarkation doorway was cracked. Replace window to the satisfaction of the attending marine inspector. 46 CFR 116.1020

Penetrations and openings in watertight bulkheads must have means to make them watertight. The bulkhead penetration for the bilge sensor wire in the lazarette was not made watertight. Make watertight to the satisfaction of the attending marine inspector. 46 CFR 171.114(a)(2)

Receptacle outlets of the type providing a grounded pole or a specific direct current polarity must be of a configuration that will not permit improper connection. The starboard-most GFCI outlet in the galley is not functioning properly. Repair to the satisfaction of the attending marine inspector. 46 CFR 120.220(c)

The owner, managing operator, or master shall report damage to a boiler, unfired pressure vessel, or machinery that renders further use of the item unsafe until repairs are made, to the OCMI at the port in which the casualty occurred or nearest the port of first arrival, as soon as practicable after the damage occurs. The vessel experienced a failure of the starboard main diesel engine's gear shift control cable. As instructed, the vessel shall proceed from Long Wharf to the BHC maintenance facility at Mystic Pier Marina to effect repairs. At all times during the transit, an engineer shall be stationed in the engine room to assume local control of clutch in/out and throttle controls of the port main diesel engine in the event of a similar failure to that engine. The operator of the vessel shall demonstrate satisfactory operation of the vessel's gear shift and throttle controls to the attending marine inspector upon completion of repairs. 46 CFR 185.208

The minimum capacity of the power fire pump shall be 50 gallons per minute at a pressure of not less than 60 pounds per square inch at the pump outlet. The pump outlet shall be fitted with a pressure gage. The fire pump was missing a pressure gage at the outlet, must install. 46 CFR 181.10-1(c)

Installation of diesel engine exhaust pipe shall be in accordance with 182.15-20. #2 SSDG has an exhaust leak on the bottom side of the manifold, must be addressed. 46 CFR 182.20-20(a)

The switchboard shall be adequately protected and provision shall be made for access thereto. The Battery distribution panel missing blocking slot to prevent accidental shocking, must be installed. 46 CFR 183.05-15(a)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of the hull structure and its appurtenances including the following: (1) Inspection of all accessible parts of the exterior of the hull. During inspection, 2 holes were found on the fuel fill containment skirt STBD side of the vessel. Repair to the satisfaction of the attending MI.

The construction of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Hull plating beneath keel coolers was not available for inspection. Remove forward keel coolers to make relevant portion of hull visible for inspection. Additionally, replace rubber bushings that have become deteriorated.

The construction of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. Marine Inspector observed significant wastage on second frame overhead, aft of engine room. Repair to the satisfaction of attending marine inspector.

An exterior lighting fixture must comply with the requirements of UL 595. Install light cover on second deck exterior light.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Marine Inspector observed a surge protector in wheelhouse with multiple electrical sources plugged into it. Remove surge protector and plug directly vessel's receptacles.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull. The vessel has a hole in the hull below the rub rail on port bow due to a pier strike. Submit repair proposal prior to commencing work on vessel and notify Sector Boston when work commences to ensure a marine inspector can attend vessel as needed.

A self priming, power driven fire pump must be installed on each vessel. The seal on the port fire pump is leaking. Repair or replace to the satisfaction of the attending marine inspector.

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads and weather deck. In the forward port section of the lazaratte, thick pieces of the deck were chipping off which contained layers of coating and metal. Conduct testing to determine thickness of hull plating in the identified location. If more than 25% wasted, crop and renew to good metal and make repairs in kind to the satisfaction of the attending marine inspector.

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads and weather deck. There is a penetration in the hull found in the #2 void between the 2nd and 3rd frame on the port side. Crop and renew. 46 CFR 176.25-5(a)

At each initial and subsequent inspection for certification, the marine inspector shall view the vessel afloat and shall conduct the following tests and inspections of the hull structure and its appurtenances: Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads and weather deck. In the forward port section of the lazaratte, thick pieces of the deck were chipping off which contained layers of coating and metal. Further examination and cleaning of the area is required to determine the condition. 46 CFR 176.25-5(a)

Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length, not more than 6 feet, securing the waterlight around the body of the ring life buoy. The lanyard was in poor condition and needs to be replaced.

An ammeter must be provided for a generator rated at 50 volts or more. The ammeter for both generators are not operable. 46 CFR 183.15-5(g)

Vsl is broadcasting no dimension information (AIS Antenna Position ABCD values equals 0) and Vsl is broadcasting no static draft information

Make STBD overhead IWO vent pipe watertight. 46 CFR 171.124 OLD "T"

Watertight door in engine room found not watertight. Make watertight.
