Upper low pressure CO2 tank gauge calibration has expired. Gauge is displaying 97%. Manufacture instructions recommend maintaining tank level at 98%. Calibrate gauge, determine accurate tank level, and adjust tank level IAW the manufacturer’s instructions as needed to the satisfaction of attending CGMI and ABS Surveyor.

Door #10 is missing a bottom hydraulic pin. Install hydraulic pin IAW the manufacturer’s instructions to the satisfaction of attending CGMI and ABS Surveyor.

Bridge smoke detection panel is displaying an error: “Error: Stores Flat Fwd”. Troubleshoot and rectify error IAW the manufacturer’s instructions to the satisfaction of attending CGMI and ABS Surveyor.

Port steering pump power failure alarm cannot be silenced in hand steering mode and is only displaying a visual alarm in NFU mode. Rectify alarm issues and demonstrate proper operation of the entire steering system to the satisfaction of attending CGMI and ABS Surveyor.

Eleven fire dampers and closures inoperable due to lack of maintenance, broken hinges, wasted gasket channels, or dry rotted gaskets. Repair all fire dampers and closures to the satisfaction of attending CGMI; a.Forepeak centerline fire damper inoperable due to lack of maintenance. b.Engine room port and starboard supply will not close and are missing screens. c.Focsle port and starboard fire damper hinges wasted – renew hinges. d.Focsle centerline fire damper gasket channel wasted – renew, repair, or replace to the satisfaction of attending CGMI and ABS Surveyor. e.Cargo deck, port side fire damper adjacent to tank #3 – renew gasket. f.4A Hold, 2nd deck, the two aft most fire dampers inoperable due to lack of maintenance. g.S3-49-2 fire damper does not close, cover and hardware removed – reinstall and demonstrate operation. h.E2-61-1 Exhaust – closure lying on deck – properly install closure.

The Class 3 watertight door would not close using the local or remote manual controls. Found the remote indicator stuck in the closed position. Prove proper operation of the Class 3 watertight door using both manual controls. Prove proper operation of remote indicator.

#2 lifeboat engine’s lube oil cooler repaired with epoxy. Make permanent repair IAW the manufacturer’s instructions to the satisfaction of attending CGMI and ABS Surveyor.

Vessel was not fully crewed during inspection. Demonstrate crew’s proficiency in firefighting, abandon ship drill, vessel control systems, and engineering upon activation to the satisfaction of CGMI.

Provide crew’s licenses and documents, updated charts, and publications upon activation to the satisfaction of CGMI upon activation.

Found cooling water piping to the starboard steering gear missing mounting hardware or brackets and/or mounted with rope. Properly mount piping using appropriate metallic hardware to the satisfaction of attending CGMI and ABS Surveyor.

Oily Water Separator (OWS) automatic three-way valve is not properly shifting from overboard to re-circ. Manufacture recommended testing solution for the Oil Content Meter (OCM) was not onboard. Prove proper operation of the OWS automatic three-way valve. Provide manufacture recommended testing solution OCM. To the satisfaction of attending CGMI and ABS Surveyor.

Found a soft patch on the overboard piping for the OWS. Renew pipe to original to the satisfaction of attending CGMI and ABS Surveyor.

Found a coffee can mounted under the vessels shaft seal used to catch oil leaking from the seal. The can is overflowing with oil and draining into the shaft alley bilge. Properly dispose of oil collected in the can and the shaft alley bilge. Monitor shaft seal for oil leaks during next underway period and provide a repair proposal to the satisfaction of attending CGMI and ABS Surveyor.

Port side hull access door aft of frame 47 has a hydraulic leak. Repair hydraulic leak and clean hydraulic oil out of door frame to the satisfaction of attending CGMI and ABS Surveyor.

Provide Owners Annual Preventative Maintenance Report and procedures for conducting condition monitoring measures for required equipment to the satisfaction of attending CGMI and ABS Surveyor.

Lighting in the emergency escape trunk in the emergency fire pump room is inoperable. Prove proper operation of lighting. 2nd Deck, stbd side, aft of frame 42, emergency escape trunk has no emergency lighting. Install emergency lighting so that the means of escape is clearly visible. To the satisfaction of attending CGMI and ABS Surveyor.

#4 Lifeboat engine has a dry rotted cool water hose and the turbo filter has fallen into the bilge. Repair IAW the manufacturer’s instructions to the satisfaction of attending CGMI and ABS Surveyor.

The AFF tank port side aft lower level engineroom is missing its vent, the sight glass valve is not operable, and the level of agent can’t be determined using the sight glass. Install an approved vent, prove proper operation of sight glass valve and prove the level of agent using the sight glass. To the satisfaction of attending CGMI and ABS Surveyor.

#3 Lifeboat launching appliance aft-most sheave bracket is severely corroded. De-scale, gauge, and repair IAW the manufacturer’s instructions as needed to the satisfaction of attending CGMI and ABS Surveyor.

#1 Lifeboat engine hydraulic starter fitting is leaking. Repair fitting IAW the manufacturer’s instructions to the satisfaction of attending CGMI and ABS Surveyor.

Port side aft lower level engine room work bench has an overhead mounted lighting fixture installed that’s plugged into an electrical outlet. Permanently wire the light to a lighting panel or remove lighting fixture to the satisfaction of attending CGMI and ABS Surveyor.

Found an opening in the lower machinery port power panel 7. Install breaker filler plate to the satisfaction of attending CGMI and ABS Surveyor.

Couldn’t determine if the main switchboard mats were dielectric. Provide documentation that installed mats are dielectric or replace with dielectric matting suitable for high voltage application to the satisfaction of attending CGMI and ABS Surveyor.

Various piping systems in the engineroom are missing support brackets and associated hardware. Repair/replace with appropriate metallic hardware to the satisfaction of attending CGMI and ABS Surveyor.

Found evidence of exhaust leaks on the starboard side SSDG. Troubleshoot, find, and repair exhaust leaks. Or provide documentation of repairs and clean all soot covered surfaces. To the satisfaction of attending CGMI and ABS Surveyor.

Starboard side engine room workshop is missing insulation overhead and on the bulkhead. Research and determine if Structural Fire Protection is required in those areas. If required install approved Structural Fire Protection insulation to the satisfaction of attending CGMI and ABS Surveyor.

36" sea water suction valve to the main circ pump inoperative due to froctured ring gear. renew vavle actuator and prove proper operation prior to sailing.

Crew unable to isolate main condenser from the sea indicating malfunction of one or more of the three 36" sea valves. Prove ability to isolate condenser for emergency repairs if needed.
