A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. Starboard MDE did not shutdown when testing the pressure switch installed in the fixed fire fighting system. 46 CFR 118.410(b)(10)

Vessels described by 46 CFR 114.110(f) must submit plans to the cognizant OCMI, in accordance with 46 CFR 115.700, for the installation and use of monitoring device(s) to ensure the wakefulness of the watchmen required in paragraph (a) of this section. (3) Be arranged to ensure proper coverage of the passenger accommodation spaces, common areas, and spaces with potential fire hazards. Several watchman alarm switches were missing and the system could not be fully tested. 46 CFR 122.410(b)(3)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Generator overspeed test did not properly secure the generators when tested. 46 CFR 115.840

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; minimize accidental personnel contact with energized parts. The following electrical installation deficiencies were identified: - Crew berthing H2 was missing light cover - crew passageway forward and aft lighting had broken covers -AC flats junction box was found not properly secured or covered and had exposed wiring 46 CFR 120.200

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. The following deficiencies were identified: - Crew berthing passageway had wires exposed and a kill switch installed. -Passenger room 107 did not have functioning PA speaker 46 CFR 116.530

Waste Receptacles. Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Waste receptacles in crew heads (mens and womens) were constructed with combustible material (plastic). 46 CFR 116.405(i)

At each inspection for certification and at every other vessel inspection, quarters, toilet and washing spaces, galleys, serving pantries, lockers, and similar spaces may be examined to determine that they are serviceable and in a sanitary condition. Galley range hood and fan had excessive grease build up that was leaking down the bulkhead and pooling up on the floor. 46 CFR 115.818

A grease extraction hood required by § 118.400 of this part must meet UL 710 “Exhaust Hoods for Commercial Cooking Equipment,” or other standard specified by the Commandant. Several holes in the side of the grease extraction hood were identified which does not meet UL 710 or NFPA 96. 46 CFR 118.425

Each vessel must be equipped with at least two operable portable battery lights located at the operating station and machinery spaces. EGen portable light was inoperable. 46 CFR 120.430

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Aft bulkhead in laundry space has through hull penetration that would allow the passage of flame and smoke. 46 CFR 116.415(a)(2)(ii)

Remote fuel shutoff stations must be marked in clearly legible letters at least 25 millimeters (1 inch) high indicating purpose of the valve and direction of operations. Egen remote fuel shutoff was not properly labeled. 46 CFR 122.608

A passenger safety bill must be posted by the master in each passenger cabin or stateroom on a vessel of more than 19.8 meters (65 feet) in length with overnight accommodations for more than 49 passengers. passenger safety bills were found missing in passenger rooms 109 and 110. 46 CFR 122.515(a)

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries. The following deficiencies were identified as missing IAW the fire control plan: - Prom deck Chief Engineer berthing was missing a PFD and PFD label - Prom Deck starboard side 3 man berthing was missing PFD - Salt room was missing PFD's. Lounge deck berthing L41 was missing PFD placard. 46 CFR 116.530

For a vessel, the construction of which was begun prior to approval of the plans and information required by paragraphs (a) and (b) of this section, the cognizant OCMI may require any additional plans and information, manufacturers' certifications of construction, testing including reasonable destructive testing, and inspections, which the OCMI determines are necessary to verify that the vessel complies with the requirements of this subchapter. Due to the recent upgrade of the fire detection system, an updated fire control plan shall be completed and approved by MSC prior to operating in 2026. Additionally, it was identified that an A class bulkhead was located on the approved fire control on the main deck in the forward stores but was not present. It appears to be an error on the approved plans as the A class bulkhead was never installed at original construction. 46 CFR 116.202(c)

Complete a survey of hull/side shell and provide a detailed drawing and plans for repairs. Due to multiple welds and scattered pits throughout.

Due to the rapid corrosion and excessive deterioration of hull and anodes a corrosion survey needs to be conducted to determine the cause of the corrosion.

24MAY24: A lighting fixture must be installed as follows: Each lighting fixture and lampholder must be fixed. A fixture must not be supported by the screw shell of a lampholder. Overhead lighting in EDG space behind the PA system was found zip tied in the overhead. 46 CFR 120.410(c)(2)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for protection against fire. Laundry room dryer ventilation was found zip tied to the overhead and attached with combustible materiald. The other end of the dryer ventilation was found with a rag inside and ventilating into the space. 46 CFR 116.100

Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. A Class deck penetration in laundry space is missing fire block. 46 CFR 116.415 (a)(2)(ii)

A door in a fire control boundary must meet the following requirements. Class A doors leading from the dining room into the galley do not close properly. 46 CFR 116.435(b)

Holdback hooks are not allowed. If installed, a hold back mechanism for a door must allow the door to be released. Door leading from galley down to the lower engineering space was found with a rope holdback. 46 CFR 116.435(c)(6)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems including inspection of each inflatable liferaft, inflatable buoyant apparatus, and inflatable lifejacket to determine that it has been serviced as required by § 185.730 of this chapter Liferafts were not onboard at time of inspection. 46 CFR 115.808(a)(4)

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. 06 life rings onboard were found with severe UV damage and cracking. 46 CFR 115.808(b)

At each initial and subsequent inspection for certification, the owner or managing operator must be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. The emergency diesel generator space CO2 system had not been serviced. 46 CFR 115.810(a)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Bridge electrical panel on aft bulkhead was missing a blank. 46 CFR 120.200(b)

Doors in A-Class fire control boundaries must meet the following additional requirements: If installed, a hold back mechanism for a door must allow the door to be released locally, upon a signal from a control space; and upon disruption of the power system. Several doors did not close properly and several could not be tested due to obstructions in and around the doors. 46 CFR 116.435(c)(6)(ii)

On a vessel of more than 19.8 meters (65 feet) in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. PA system in "MARIA" rooms were inoperable. 46 CFR 121.610(b)

Each floating waterlight must be approved in accordance with § 161.010 in subchapter Q of this chapter, or other standard specified by the Commandant. Waterlight had incorrect batteries installed not IAW approval letter. 46 CFR 117.70(d)(1)

Fire extinguishing equipment installed on a vessel in excess of the requirements of §§ 118.400 and 118.500 must be designed, constructed, installed, and maintained in a manner acceptable to the Commandant. Vessels installed sprinkler system which is excess equipment installed but not required has not been serviced IAW NFPA 13. 46 CFR 118.120

Hot water tank in engine room was missing the cover for the electrical connection/controller box. 46 CFR 120.200

Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. Several overhead lights in engine room and emergency diesel generator room were mounted with zip ties and bailing wire. 46 CFR 120.220

Distribution panels and switchboards that are accessible from the rear must be constructed to prevent a person from accidentally contacting energized parts. #3 Generator was being serviced and the rear cover was removed. Replace cover while not actively being serviced.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Multiple removable deck plates throughout the lower decks including engine room were not secured and missing mounting hardware.

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment. Vessels fixed fire protection system in the engine room, emergency diesel generator space and galley had not been serviced since Feb 2022. 46 CFR 115.810

On a vessel on which the number of passengers permitted on upper decks is limited by stability criteria, as indicated by the vessel's stability letter, the maximum number of passengers allowed on an upper deck must be indicated by a durable marking of at least 25 millimeters (1 inch) numbers and letters at the entranceway to that deck. Aft stairs leading to the upper deck was not properly marked with maximum passenger capacity. 46 CFR 122.602

Each vessel must be provided with a visual and audible alarm at the operating station. Bilge high water alarm system was not providing an audible alarm at the operating station. 46 CFR 119.530

Complete but simple instructions for the operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control and in the space where the extinguishing agent cylinders are stored. If the storage cylinders are separate from the protected space, the instructions must also include a schematic diagram of the system and instructions detailing alternate methods of releasing the extinguishing agent should the local manual release or stop valve controls fail to operate. Simple instructions and schematic diagram of fixed gas fire system in engine room and emergency diesel generator space were missing. 46 CFR 122.612

Each vessel that relies on electricty to power vital systems must be arranged so that the loads can be energized from two sources of electricty. Two generators are currently unable to be paralleled to supply adequate load, the third genertor has an excessive oil leak. Provide tech report and demonstrate proper operation prior to movement. 46 CFR 120.310(a)

Electrical equipment on a vessel must be installed and maintained to provide services necessary for safety under normal and emergency conditions. The #3 generator was unable to maintain proper voltage. Demonstrate proper operation of the #3 generator on load to the satisfaction of the attending MI. 46 CFR 120.200(a)

Each survival craft must be secured to the vessel by a painter with a float-free link. Two liferafts on the port side were secured to the cradle with line and the float free link was not installed. The hydrostatic release was properly installed, cleared on the spot. 46 CFR 117.130(a)(1)

Paint and flammable liquid lockers must be constructed of steel or equivalent material, or wholly lined with steel or equivalent material. A fiberglass deck box was located on the main deck aft that was filled with paint and other flammable materials. 46 CFR 116.405(e)

Fire load calculations must be submitted by the owner for review to the Marine Safety Center when: (1) A space is designated as a low risk accommodation space by the owner; or (2) The cognizant OCMI determines, based on the quantity of combustibles, that the fire load present in a high risk accommodations space may exceed 37.5 kg (7.5 pounds) of combustibles per square meter (square foot) of deck area. Space forward of passenger cabins on the lounge deck as well as the starboard forward passenger cabin (aka the Salt Room) had linens, towels and other large quantities of combustibles stored in these spaces. 46 CFR 116.427(a)

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show compartment titles and hazard classification of accommodation and service spaces. Starboard main deck passenger cabin has been converted to a "Salt Room". Fire control plan does not reflect on changes made to this space. Deficiency to be corrected prior to beginning of next operating season in 2023. 46 CFR 116.530

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules, except that a vessel of more than 19.8 meters (65 feet) in length must also have navigation lights that meet UL 1104, “Marine Navigation Lights,” or other standard specified by the Commandant. All around anchor light was inoperable. 46 CFR 120.420

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. The starboard galley ventilation did not completely seal when closed. 46 CFR 116.100

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. Beverage C02 bottles in lounge are unsecured. 46 CFR 122.100

Tubular gauge glasses, if fitted to diesel fuel tanks, must be of heat resistant materials, adequately protected from mechanical damage, and provided at the tank connections with devices that will automatically close in the event of rupture of the gauge or gauge lines. EDG fuel tank has a plastic gauge and does not have a device installed to automatically close in the event of rupture. 46 CFR 119.440(a)(7)

UT tech report found multiple areas of wastage that is above 25% and in some areas as much as 42%. Crop and renew to original scantling. The areas of wastage according to the report were found: -near the keel between frame 13 and running to frame 19 on both port and starboard side -near the keel between frame 22 to frame 26 -On the port side bottom midships between frame 22-frame 24. -Near the keel, port side between frame 29-31 -Between port frame 32 and 33 -Between port frame 33 and 36 -Between port frame 37 and 38 -Between port frame 39 and 42 -Between Starboard frame 30 and 32 -Between starboard frame 33 and 36 -Between starboard frame 38 and 39 -Between starboard frame 39 and 42 -Port side between frame 59-60

In the outboard most area of the bottom of the hull at Port frame 41, UT shots found 30% wastage. Due to the placement of blocks and shipyard restrictions, welding to crop and renew the wastage is not possible during this dry dock. Crop and renew to original scantling during next dry dock.

Vessel has had considerable amount of hull replaced within the past 10 years and the port anchor is no longer on board. There is a concern of a total aggregate vessel weight change of more than 2% since the last stability test. Provide evidence that the vessel's weight has not undergone total aggregate change of more than 2%.

Provide complete instructions for operation of all fixed gas fire extinguishing systems iaw. 46CFR122.612 at or near each pull box and stop valve control. also include a schematic diagram of the system and instructions detailing alternate methods of releasing agent.

While the vessel is engaged in towing the vessel must be controlled by a licensed Master or Mate of towing. Vessel has until April 1st 2022 to man vessel with licensed Master and Mate of towing.

Vessel must provide updated stability calculations to include towing to Coast Guard Marine Safety Center to update vessels stability letter by April 1st 2022

Vessel shall provide correct light and day shape configuration for the tow being conducted prior to towing.

Vessel shall start and maintain records for towline and terminal gear used for towing astern or alongside prior to towing

Vessel has two fractured stiffeners on the forward bulkhead of the potable water tank, starboard side. Repair cracked welds no later then May 1, 2020.

Posted Emergency Instructions and Station Bill fail to address procedures for rough seas and addressing the fitting of daedlight covers on main deck windows as required by COI's routes and conditions. Update Emergency Instructions and Station Bill as appropriate.
