A space containing propulsion machinery must be equipped with a fixed gas fire extinguishing system. in compliance with 181.410. Vessel's CO2 system shutdowns did not function as designed. Correct CO2 shutdown and notify local OCMI to conduct operational test of system.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found in engine compartment, laz and fore peak. Remove dead end wiring and update electrical plans.

Spanner wrenches must be provided where a 40 millimeter (1.5 inch) diameter fire-hose is required by § 181.320(b). Spanner wrench missing. Provide spanner wrenches.

An alarm for a fixed gas fire extinguishing system must be clearly and conspicuously marked “WHEN ALARM SOUNDS-VACATE AT ONCE. CARBON DIOXIDE BEING RELEASED”. Where a different extinguishing agent is installed, that agent shall be marked in place of “carbon dioxide.” (g) Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked as follows: (1) Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.”. (2) Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” The reference to wintergreen scent may be omitted for carbon dioxide systems not required to have odorizing units and not equipped with such units. (3) Spaces into which carbon dioxide might migrate “CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. DISCHARGE INTO NEARBY SPACE CAN COLLECT HERE. WHEN ALARM OPERATES OR WINTERGREEN SCENT IS DETECTED VACATE IMMEDIATELY.”

All griddles, broilers, and deep fat fryers must be fitted with a grease extraction hood in compliance with 181.425. A grease extraction hood must meet UL 710 or other standard specified by the Commandant. A grease extraction hood must be equipped with a dry or wet chemical fire extinguishing system meeting NFPA 17 or other standard specified by the Commandant, and must be listed by an independent laboratory recognized by the Commandant. Install grease extraction hood equipped with a dry or wet chemical fire extinguishing system to the satisfaction of an attending marine inspector at next credit drydock, not later than Mar 1, 2026.

Vessel must comply with master key control requirements outlined in 46 USC 3106. Provide evidence of compliance within 30 days.

Vessel reported a loss of propulsion on the STBD Shaft and sent a deckhand to investigate and found the shaft to be cracked. Vessel is to provide a repair proposal and USCG Marine Inspector shall follow-up on the repairs and will perform testing prior to clearing DEF.

The marine inspector may require any part or all of the propeller shafting to be drawn for examination of the shafting and stern bearing of a vessel whenever the condition of the shafting and bearings are in question. During the inspection it was discovered that the forward and aft shaft bearings have been worn down under acceptable limits. Also discovered the starboard shaft had a wear ring near the forward bearing. Replace all 4 bearing with new one, have the starboard shaft repaired and conduct an Nondestructive test. Also, conduct a realignment for both shafts and provide report to the local OCMI.

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Found dead end wires in the Fore peak, Lazarette and forward of the port MDE in the E/R. Remove or place in junction.

Passenger compartment stairwell aft electrical panel circuit breaker missing its cover. Install breaker cover to prevent electrical shock.

Each ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018. Current ring life buoy arrangement did not have the retroreflective material attached. Must correct deficiency to satisfaction of the Coast Guard Marine Inspector.

Replaced wasted and leaking fuel oil filter for the port propulsion engine.

Prove proper operation of bilge pumping system.

Vessel has been identified as having various A/C electrical modifications since initial certification. Sector San Diego does not have vessels electrical plans on file or current installations do not match what we have on file. Provide a one-line electrical drawing for all A/C switchboards and distribution panels. Include all circuit loads, conductor sizes, and over current protection.

All escape hatches and emergency exits used as means of escape must be marked on both sides in clearly legible letters and be well lit during loss of power. Emergency exit sign was not visible in lower berthing spaces during loss of power. Must rectify to the satisfaction of the CG Marine Inspector.

All electrical outlets must be adequately protected to prevent personnel injury or associated hazard to the vessel. Receptacle outlet under helm station was missing cover. Must install cover to satisfaction of CG Marine Inspector.

Provide coast pilot , light list, and tide tables that are current.

Lead Captain/owner: provide current medical/physical endorsement for STCW and Nationl medical certificat.

Provide suitable mounting of Raw Water strainers as designed.

Provide marine grade fused for both MDE

Post DAPI ordnances and company policy info.

No sail due to Lapse in hull exam

Prove proper operation of both generators. Also prove shutdown when CO2 system activated.

Prove proper operation of bilge alarms in forepeak and forward hall.

Provide serviceable distress signals.

Prove proper operation of stern light.

Conduct drills when fully manned in presence of Coast Guard Marine Inspector.

Prove proper operating pressure of fire pump.

Properly dead-end loose wires in Engine Room.

Properly connect wires in Engine Room.

Provide 03 serviceable life ring buoys properly label and outfitted

Seperate child sized and adult sized life jackets.

Replace retaining cover for CO2 pull station.

Permanently affix vessel's documentation number to the interior structure of the vessel.

Properly post vessel stability letter under a suitable transparent material in the pilothouse

Properly post COI under a suitable transparent material in the passenger space.

Provide updated first aid kit

Provide updated coast pilot and light list on-board the vessel.

Repair pitted area in port quarter/port laz.

Remove spigot patch/doubler and repair holed area IWO port side inboard transducer.

Repair fuel tank piping located on aft E/R bulkhead. Piping has a soft patch fitted currently
